带量采购模式下某院质子泵抑制剂应用情况分析
Analysis on the Application of Proton Pump Inhibitors in A Hospital under the Mode of Volume-based Procurement
【摘要】 目的 分析药品“4+7”带量采购对医院质子泵抑制剂(PPIs)应用的影响。方法 选取该院THIS系统中“4+7”带量采购前(2019年4月24日~2020年4月23日)和带量采购后两年(2020年4月24日~2022年4月23日)质子泵抑制剂药品生产企业、价格、消耗量、使用频度、销售金额等数据,比较各类质子泵抑制剂的供应目录、销售价格、用药频度(DDDs)、销售金额、限定日费用(DDDc)、排序比(B/A)。结果 带量采购政策执行两年后我院PPIs降价品规数达到12个,降价幅度在7.02%~92.25%之间。总DDDs呈逐年下降趋势,其中口服剂型的DDDs波动增长,同比带量采购前增幅5.21%,注射剂型的DDDs逐年下降,同比带量采购前最大降幅51.26%。总DDDc呈逐年下降趋势,最大降幅45.52%。PPIs原研品种使用占比持续下降,低于仿制品种。结论 带量采购使PPIs使用结构发生变化,减轻患者用药负担。
【Abstract】 OBJECTIVE To analyze the influence of “4+7” volume-based procurement on the application of proton pump inhibitors(PPIs) in hospitals.METHODS Select the data of PPI manufacturers, price, consumption, defined daily dosages and sales amount before the “4+7” volume-based procurement(April 24,2019-April 23,2020) and two years after the volume-based procurement(April 24,2020-April 23,2022) in THIS system of the hospital.The supply list, selling price, defined daily dosages(DDDs),selling sum, defined daily dose cost(DDDc) and ranking ratio(B/A) of various PPIs were compared.RESULTS After two years of implementation with volume-based procurement, the price reduction of PPIs in our hospital reached 12 typrs, and the price reduction range was 7.02%-92.25%.The total DDDs showed a downward trend year by year.Among them, DDDs of oral dosage forms fluctuated and increased by 5.21% compared with that before the volume-based procurement, while the DDDs of injectable dosage form decreased year by year with the largest decrease of 51.26% compared with that before volume-based procurement.The total DDDc showed a decreasing trend year by year, with a maximum decrease of 45.52%.The proportion of use of original PPIs decreased continuously, which was lower than that of generic varieties.CONCLUSION Volume-based procurement can change the use structure of PPIs and reduce the phamaceutical financial burden of patients.
