【摘要】 目的 通过建立与改善我院门急诊事前审方模式，提高处方合格率及门急诊合理用药水平。方法 建立事前审方合理用药系统，比较事前审方工作开展前后的处方合格率，分析不合理处方类型分布及干预成功率。并针对人工审核效率低、患者平均就诊时间延长、药师判定处方有偏差等问题，实践改善措施。结果 通过审方系统的规则自维护、处方排队时间设置、构建药师培训体系、借助微信企业号实现实时沟通，假阳性问题、审方时间明显减少，审方效率提高。结论 我院建立的门急诊事前审方模式切实可行，保障了患者用药的合理性和安全性。更多还原

【Abstract】 OBJECTIVE Through the establishment and improvement of our hospital outpatient and emergency prescription review mode, improve the prescription qualified rate and the level of rational drug use in outpatient and emergency department.METHODS To establish the system of rational drug use, and to compare theprescription qualification rate before and after the Prior-review work.To analyze the distribution of irrational prescription types and the success rate of intervention.In view of the problems such as low efficiency of manual prescription review, prolonged average visiting time of patients, deviation of pharmacists′ judgment of prescription, the improvement measures were put forward.RESULTS Through the self maintenance of the rules, the setting of the queuing time, the professional training of pharmacists, and wechat enterprise, the false positive problems and the time of prescription review are significantly reduced, and the efficiency of prescription review is improved.CONCLUSION The outpatient and emergency prescription review mode of Our hospital is feasible, which ensures the rationality and safety of patients′ medication.更多还原
