【摘要】 目的 探索腰大肌指数与骨质疏松性椎体压缩骨折(osteoporotic vertebral compression fracture,OVCF)患者保守治疗预后的相关性。方法 回顾性分析2017年1月至2021年1月，于我院门急诊接受保守治疗的63例OVCF患者，根据L3椎体中间层面的腰大肌指数，分为低骨骼肌质量组和非低骨骼肌质量组。分别比较两组患者的年龄、性别、体质量指数(body mass index,BMI)等基础信息，以及两组患者在骨折时、骨折后1个月、3个月和6个月的椎体塌陷率和腰背部疼痛程度，分析骨骼肌质量与OVCF患者保守治疗的预后相关性。结果 低骨骼肌质量组与非低骨骼肌质量组在性别、年龄、BMI、骨密度以及骨折节段方面组间差异无统计学意义。低骨骼肌质量组的患者保守治疗后1个月(P=0.006)、3个月(P <0.001)和6个月(P <0.001)的椎体塌陷率的组比非低骨骼肌质量组更严重，保守治疗6个月(P <0.001)后的腰背痛恢复效果更差。结论 在OVCF患者保守治疗6个月的随访期间，低腰大肌指数的患者具有更差的预后，椎体塌陷进展更快，残留腰背痛更显著。更多还原

【Abstract】 Objective To explore the relationship between psoas muscle index( PMI) and prognosis of conservative treatment of osteoporotic vertebral compression fracture( OVCF). Methods A retrospective study of 63 OVCF patients who received conservative treatment was conducted from January 2017 to January 2021. According to PMI, all were divided into low skeletal muscle mass group and non-low skeletal muscle mass group. The age,gender, BMI, vertebral body collapse rate and the low back pain at the time of fracture, 1 month after fracture, 3 months after fracture, and 6 months after fracture were compared between the two groups. Results There were no significant differences in gender, age, BMI, bone mineral density and fracture segment between the two groups. Compared with the non-low skeletal muscle mass group, the low skeletal muscle mass group had worse vertebral body collapse rate 1 month( P = 0.006), 3 months( P < 0.001) and 6 months( P < 0.001) after conservative treatment. Worse low back pain recovery was observed after the conservative treatment of 6 months( P < 0.001). Conclusions During the 6-month follow-up of conservatively treated OVCF patients, patients with low PMI have worse prognosis, faster progression of vertebral collapse, and severer residual low back pain.更多还原
