脑缺血/再灌注导致神经元损伤机制的研究进展
Research progress on the mechanism of neuronal injury caused by cerebral ischemia/reperfusion
【摘要】 卒中是一种高发病率、高致残率、高病死率的疾病。目前缺血性脑卒中的治疗方法主要为静脉溶栓,但事实表明血流的复灌会进一步加重神经元的损伤,即缺血再灌注损伤(IRI)。脑缺血/再灌注损伤涉及氧自由基增多、炎症因子释放增加、兴奋性氨基酸毒性、血脑屏障损伤、细胞内钙离子超负荷等多种病理生理过程,研究其潜在的机制有助于保护神经元免受损伤,改善脑缺血再灌注后患者的症状及预后。本文就脑缺血/再灌注导致神经元损伤的具体机制进行综述,旨在为临床药物开发提供新视野。
【Abstract】 Stroke is a disease with high morbidity, high disability rate and high mortality. At present, the treatment of cerebral ischemic stroke is mainly intravenous thrombolysis, but the fact shows that the reperfusion of blood flow will further aggravate the neuronal injury, namely, ischemia reperfusion injury(IRI). IRI involves a variety of pathophysiological processes(e.g. oxidative stress, calcium overload, release of circulating inflammatory factors, destruction of blood-brain barrier, excitotoxicity of excitatory amino acids, etc.). The study on its potential mechanism is helpful to protect neurons from injury and improve the symptoms and prognosis of patients with cerebral ischemic reperfusion. This article summarizes the specific mechanism of neuronal injury caused by cerebral ischemia/reperfusion, so as to provide a new perspective for clinical drug development.
