【摘要】 目的 探讨尿中阿尔茨海默病相关神经丝蛋白（AD7c-NTP）作为生物标志物在早期诊断阿尔茨海默病（AD）的应用价值。方法 纳入2021年12月～2022年7月在我院老年科住院的48例AD患者为AD组，29例血管性痴呆（VD）患者为VD组，同时入选65例正常老年人群为正常对照组作为研究对象，利用化学发光法检测患者尿中的AD7c-NTP浓度。结果 AD组尿AD7c-NTP的平均水平为（1.494±0.612）ng/mL,VD组尿AD7c-NTP的平均水平为（1.119±0.991）ng/mL，对照组尿AD7c-NTP的平均水平为（0.478±0.386）ng/mL。尿AD7c-NTP诊断早期AD的灵敏度、特异度分别为93.8%和84.6%，最佳截断值为0.685，曲线下面积（AUC）为0.947,95%CI(0.905,0.989)。尿AD7c-NTP诊断早期VD的灵敏度、特异度分别为75.9%和69.2%，最佳截断值为0.445，曲线下面积（AUC）为0.722,95%CI(0.596,0.848)。结论 尿AD7c-NTP在AD的早期诊断中应用价值高，利用化学发光法检测，以0.685ng/mL作为诊断临界值对应的敏感性和特异性最大，可作为早期诊断AD的重要生物学标记物之一。更多还原

【Abstract】 Objective To explore the value of Alzheimer’s disease associated neurofilament protein(AD7c-NTP in urine as a biomarker in the early diagnosis of Alzheimer’s disease(AD). Methods 48 AD patients hospitalized in the geriatric department of Chenzhou Psychiatric Hospital from December 2021 to July 2022 were included as AD group,29vascular dementia(VD)patients as the VD group, and 65 normal elderly people as the normal control group were selected as the study object. The AD7c-NTP concentration in the urine of the patients was detected by chemiluminescence method.Results The average level of AD7c-NTP was(1.494±0.612)ng/mL in AD group,(1.119±0.991)ng/mL in VD group, and(0.478±0.386)ng/m L in control group. The sensitivity and specificity of urine AD7c-NTP in the diagnosis of early AD were 93.8% and 84.6%,respectively.
