肿瘤患者ABCB1基因多态性与紫杉醇疗效及药物不良反应的相关性研究
Research on the correlation between ABCB1 genes polymorphism in tumor patients and the efficacy and adverse drug reactions of paclitaxel
【摘要】 目的 探讨肿瘤患者腺苷三磷酸结合盒B家族成员1(ABCB1)基因多态性与紫杉醇疗效及药物不良反应的相关性。方法 选取接受紫杉醇化疗的肿瘤患者作为研究对象,检测其ABCB1 T3435C、C1236T、G2677T/A基因型,并将患者分为野生型组和突变型组。2个周期后,对疗效和药物不良反应进行评估,分析患者的疗效及药物不良反应和ABCB1基因多态性的相关性。结果 67例肿瘤患者中,ABCB1 G2677T/A基因的野生型组的有效率(69.23%)高于突变型组(25.93%),差异有统计学意义(P<0.05);在药物不良反应指标中,ABCB1 G2677T/A基因的野生型组的谷丙转氨酶(21.42±6.93)U·L-1低于突变型组(28.80±17.96)U·L-1,两者存在显著性差异(P<0.05)。而ABCB1 C3435T、C1236T基因的野生型组、突变型组之间的疗效和药物不良反应均不存在显著性差异(均P>0.05)。结论 ABCB1 G2677T/A基因多态性与紫杉醇的疗效和肝毒性具有相关性,而ABCB1 C3435T、C1236T基因多态性与紫杉醇的疗效和药物不良反应均不具有相关性。
【Abstract】 Objective To investigate the relationship between adenosine triphosphate-binding cassette subfamily B member 1(ABCB1) gene polymorphism and the efficacy and adverse drug reactions of paclitaxel in tumor patients. Methods Tumor patients receiving paclitaxel chemotherapy were selected as research objects, and their ABCB1 T3435C, C1236T, G2677T/A genotypes were detected, and the patients were divided into wild type group and mutant group. After 2 cycles, the efficacy and adverse drug reactions were evaluated, and the correlations between the efficacy and adverse drug reactions and ABCB1 gene polymorphism were analyzed. Results Among 67 tumor patients, the effective rate of ABCB1 G2677T/A gene in the wild type group(69.23%)was higher than that in the mutant group(25.93%), with a significant difference between them(P<0.05). In the index of adverse drug reactions, glutamic-pyruvic transaminase(21.42±6.93)U·L-1 in the wild type group of ABCB1 G2677T/A gene was lower than that in the mutant group(28.80±17.96)U·L-1, with significant difference between the two groups(P<0.05). There were no significant differences in efficacy and adverse drug reactions between wild type and mutant ABCB1 C3435T and C1236T gene groups( P > 0. 05). Conclusion ABCB1 G2677T/A gene polymorphism is associated with the efficacy and hepatotoxic adverse drug reaction of paclitaxel,while ABCB1 C3435T and C1236T gene polymorphisms were not associated with the efficacy and adverse drug reactions of paclitaxel.
