【摘要】 目的:分析袋状缝合抬高臀沟术联合中药外敷对骶尾部藏毛窦术后创面愈合的影响。方法:将藏毛窦并行手术治疗的患者66例随机分为对照组和治疗组，每组33例。2组患者均采用袋状缝合抬高臀沟术治疗，术后治疗组患者应用中药止痛促愈液外敷创面，对照组患者应用复方黄柏液涂剂外敷创面，比较2组患者临床疗效，术后创面渗出评分、疼痛评分、创口愈合时间以及肿瘤坏死因子-α(TNF-α)和表皮生长因子(EGF)水平变化。结果:治疗组患者总显效率高于对照组(P<0.05);2组患者术后1 d创面渗出评分、疼痛评分以及TNF-α和EGF水平比较差异无统计学意义(P>0.05),术后7 d和14 d创面渗出评分、创面疼痛评分、血清TNF-α水平治疗组明显低于对照组(P<0.05),术后7 d和14 d血清EGF水平治疗组明显高于对照组(P<0.05);治疗组患者创面愈合时间明显短于对照组(P<0.05)。结论:袋状缝合抬高臀沟术联合中药外敷治疗骶尾部藏毛窦可减轻患者创面渗出、疼痛，降低炎症因子水平，促进创面愈合，疗效显著。更多还原

【Abstract】 Objective To investigate the influence of bag-like suture & Bascom cleft lift procedure combined with externally dressing with TCM on postoperative wound-surface healing of pilonidal sinus(PS). Methods Randomly divided the 66 PS patients treated in author’s hospital and received bag-like suture & bascom cleft lift procedure into treatment group(33 cases, after surgery externally dressing TCM-ZhitongCuyu Ye on wound-surface) and control group(33 cases, instead of TCM-compound Huangbai Ye on wound-surface);then, compared both groups’ clinical effect, the ratings on wound-surface exudation and pain, wound healing time, as well as the levels change of TNF-α and EGF. Results In total effective rate treatment group was higher than control group(P<0.05).At 1 day after surgery in the rating on wound-surface exudation and pain, as well as in the levels of TNF-α and EGF there was no statistical difference between the two groups(P>0.05),but, at 4,14 days after surgery, in above ratings and the level of TNF-α,in the level of EGF treatment group was lower, higher than control group(P<0.05);the wound-surface healing time of treatment group was shorter than that of control group(P<0.05). Conclusion Bag-like suture & Bascom cleft lift procedure combined with TCM for externally dressing in the treatment of PS can relieve patients’ wound-surface exudation and pain, reduce the level of inflammatory factors, and promote wound-surface healing, which its efficacy is significant.更多还原
