不同AMH水平的PCOS患者卵巢反应性及与体外受精—胚胎移植结局的关系
Ovarian reactivity in PCOS patients with different AMH levels and its relationship to outcomes of in vitro fertilization-embryo transfer
【摘要】 目的 探究不同抗苗勒管激素(AMH)水平的多囊卵巢综合征(PCOS)患者卵巢反应性与体外受精—胚胎移植结局的关系。方法 选取88例PCOS患者,均接受体外受精—胚胎移植,均在取卵促排前进行AMH检测,结合AMH检测结果分为高AMH组(34例)与低中AMH组(54例)。两组患者均进行常规生化检查、卵巢反应性评估、体外受精—胚胎移植结局等相关指标统计,并比较妊娠结局。结果 高AMH组和低中AMH组在窦卵泡计数、睾酮、基础垂体生成素、促性腺激素(Gn)用量、优胚率比较,差异有统计学意义(P<0.05),两组患者的基础雌二醇、基础卵泡刺激素、Gn天数、扳机日雌二醇、获卵数、优质卵泡数、受精率、临床妊娠率比较,差异无统计学意义(P>0.05)。结论 高AMH水平会影响PCOS患者的卵巢反应性,并对体外受精—胚胎移植结局造成一定影响。
【Abstract】 Objective To explore the relationships between ovarian reactivity and outcomes of in vitro fertilizationembryo transfer in polycystic ovary syndrome(PCOS) patients with different levels of anti-Mullerian hormone(AMH). Methods 88 patients with PCOS were selected, all of whom underwent in vitro fertilization and embryo transfer. AMH detection was performed before egg extraction and ovulation promotion. They were divided into high AMH group(34 cases) and low and medium AMH group(54 cases) according to the results of AMH detection. Both groups underwent routine biochemical examination, ovarian reactivity assessment, in vitro fertilization-embryo transfer outcome and other related indicators statistics, and compared the pregnancy outcome. Results There were statistically significant differences in sinus follicle count, testosterone, BPT, gonadotropin(Gn) dosage and optimal embryo rate between the high AMH group and the low and middle AMH group(P<0.05). The basal estradiol, basal follicle stimulating hormone, Gn days, trigger day estradiol, number of eggs obtained, number of high-quality follicles, fertilization rate and clinical pregnancy rate of the two groups were compared. There was no significant difference(P>0.05). Conclusion High AMH level may affect the ovarian reactivity of PCOS patients and thereby affect the outcome of in vitro fertilization-embryo transfer.
