维生素D缺乏与多囊卵巢综合征患者激素代谢的相关性分析
Correlation analysis between vitamin D deficiency and hormone metabolism in patients with polycystic ovary syndrome
【摘要】 目的 分析多囊卵巢综合征(PCOS)患者维生素D与激素代谢的相关性,为临床进一步认识和治疗PCOS提供新思路。方法 选取本院156例PCOS患者为研究对象,分为维生素D缺乏组(≤20 ng/ml)74例、维生素D非缺乏组(>20 ng/ml)82例,比较两组的激素水平,分析维生素D缺乏与激素代谢的相关性。结果 维生素D缺乏组抗苗勒管激素、黄体生成素、黄体生成素/卵泡刺激素、睾酮、脱氢表雄酮硫酸酯水平显著高于维生素D非缺乏组(P<0.05);Logistic回归分析显示,抗苗勒管激素、睾酮和脱氢表雄酮硫酸酯是PCOS患者维生素D缺乏的影响因素(P<0.05);Pearson相关分析结果显示,PCOS患者维生素D与抗苗勒管激素、黄体生成素、黄体生成素/卵泡刺激素、睾酮和脱氢表雄酮硫酸酯呈负相关(P<0.05)。结论 维生素D缺乏的PCOS患者较非缺乏者排卵异常和高雄激素血症发生率更高,应重视监测PCOS患者的维生素D水平,并对缺乏者进行合理补充,以保障女性生育健康。
【Abstract】 Objective Analyzing the correlation between vitamin D and hormone metabolism in patients with polycystic ovary syndrome(PCOS) provides new ideas for further clinical understanding and treatment of PCOS. Methods 156 PCOS patients in our hospital were selected as the study subjects, divided into a vitamin D deficiency group(≤20 ng/ml) of 74 cases and a vitamin D non deficiency group(>20 ng/ml) of 82 cases. The hormone levels of the two groups were compared and the correlation between vitamin D deficiency and hormone metabolism was analyzed. Results The results showed that the levels of anti mullerian hormone, luteinizing hormone, luteinizing hormone/follicle stimulating hormone, testosterone, dehydroepiandrosterone sulfate in the vitamin D deficient group were significantly higher than those in the non vitamin D deficient group(P <0.05); Logistic regression analysis showed that anti-Mullerian hormone, testosterone, and dehydroepiandrosterone sulfate were the influencing factors for vitamin D deficiency in PCOS patients(P<0.05); Pearson correlation analysis showed that vitamin D in PCOS patients was negatively correlated with anti-Mullerian hormone, luteinizing hormone, luteinizing hormone/follicle stimulating hormone,testosterone, and dehydroepiandrosterone sulfate(P <0.05). Conclusion PCOS patients with vitamin D deficiency have a higher incidence of ovulation abnormalities and hyperandrogenism compared to non deficient individuals. It is necessary to pay attention to monitoring the vitamin D levels of PCOS patients and provide reasonable supplementation for those with vitamin D deficiency to ensure female reproductive health.
