【摘要】 目的 探讨院内外一体化饮食运动指导用于妊娠期糖尿病孕妇的治疗效果。方法 选取96例妊娠期糖尿病孕妇，根据随机抽样法分为对照组与观察组，每组48例。观察组由营养科医师对孕妇进行专业的饮食运动指导，并进行院外的跟踪观察指导，对照组由产检医师对孕妇进行常规孕期检查，交代妊娠期糖尿病的注意事项，以及给予常规饮食指导。观察两组妊娠结局、妊娠期并发症及围产儿并发症发生情况。结果 观察组剖宫产率52.08%明显低于对照组的72.92%，阴道分娩率47.92%高于对照组的27.08%，差异有统计学意义（P<0.05）；对照组发生2例早产，观察组未发生；观察组巨大儿发生率低于对照组，差异有统计学意义（P<0.05）。观察组孕妇贫血、妊高症、胎儿生长受限、胎膜早破、羊水过多、过少等总发生率低于对照组，差异有统计学意义（P<0.05）。结论 院内外一体化饮食运动指导用于妊娠期糖尿病孕妇，能够降低剖宫产率、巨大儿发生率、孕妇妊娠期并发症发生率，值得临床广泛应用。更多还原

【Abstract】 Objective To explore the therapeutic effect of integrated diet and exercise guidance for pregnant women with gestational diabetes. Methods 96 pregnant women with gestational diabetes were selected and divided into control group and observation group according to random sampling, with 48 cases in each group. The observation group was given professional diet and exercise guidance by the nutritionist and follow-up observation guidance outside the hospital. The control group was given routine pregnancy examination by the obstetrician, explaining the precautions for gestational diabetes and giving routine diet guidance.Pregnancy outcome, pregnancy complications and perinatal complications were observed in the two groups. Results The cesarean section rate of observation group was 52.08% significantly lower than that of control group(72.92%), and the vaginal delivery rate of 47.92% was higher than that of control group(27.08%), the difference was statistically significant(P<0.05). Preterm birth occurred in 2 cases in the control group, but not in the observation group. The incidence of macrosomia in observation group was lower than that in control group, and the difference was statistically significant(P<0.05). The total incidence of anemia, pregnancy-induced hypertension, fetal growth restriction, premature rupture of membranes, excessive and oligoamniotic fluid in observation group was lower than that in control group, and the difference was statistically significant(P<0.05). Conclusion Integrated diet and exercise guidance in and out of hospital for pregnant women with gestational diabetes can reduce the rate of cesarean section, macrosomia and pregnancy complications, and is worthy of clinical application.更多还原
