肠-肺轴以及粪菌移植在脓毒症中的作用
Function of gut-lung axis and fecal bacteria transplantation in sepsis
【摘要】 肠道作为机体最大的微生物储存库,其微生态平衡与机体的健康息息相关,正常的肠道菌群不仅在促进营养物质消化吸收、阻止病原菌定植等方面发挥重要作用,同时也具备保护胃肠屏障和人体免疫系统健康的作用。脓毒症作为重症病人最常见的致死因素之一,其发生机理错综复杂,不仅涉及感染、免疫、神经内分泌等方面,还与肠道微生态平衡的破坏密切相关,同时严重脓毒症时,肠道作为最先受累的器官,会出现肠道黏膜屏障受损,肠道菌群紊乱并易位,反过来加重全身感染,加速多器官功能衰竭的进展,其中肺是肠道菌群易位后首先累及的脏器,肠-肺轴作为肠道与肺部之间的联系轴,其提出为通过调整肠道微生态平衡从而改善脓毒症肺部损害提供了新的可能。
【Abstract】 As the largest reservoir of microorganisms, the intestinal microecological balance are closely related to the health of the body, the normal gut microbiota not only plays an important role in promoting the digestion and absorption of nutrients and preventing the colonization of pathogenic bacteria, but also maintains the intestinal barrier and the stability of immune function. Sepsis is one of the most common causes of death in severe patients, its pathogenesis is complex and variable, involving not only infection, immunity and neuroendocrine, but also the destruction of intestinal microecological balance. At the same time, when severe sepsis occurs, the intestine, as the first organ to be suffered, will have a damaged intestinal mucosal barrier, intestinal flora imbalance and translocation, which in turn will aggravate the systemic infection and accelerate the progress of multiple organ failure. The lung is the first organ affected by intestinal bacterial translocation, and the gut-lung axis is the connection axis between the gut and the lung, the proposal opens up new possibilities for modifying intestinal balance of nature to ameliorate lung damage in sepsis.
