【摘要】 目的：探讨维持性血液透析(MHD)患者自体动静脉内瘘术后束臂功能锻炼的应用效果。方法：选取了2019年1月-2020年12月在宜昌市中心人民医院行MHD治疗的106例患者为研究对象，按照随机数字表法分为对照组(n=53)和观察组(n=53)。对照组实施常规握拳锻炼，观察组实施束臂功能锻炼。比较两组患者内瘘成熟率、内瘘超声检查结果、内瘘使用情况及内瘘相关并发症发生率。结果：术后2个月，观察组内瘘血流量、内瘘血管直径均大于对照组，内瘘血管距皮深度、内瘘首次使用时间、拔针后止血时间均低于对照组(均P<0.05)。观察组内瘘临床成熟率(56.60%vs 35.85%)、内瘘超声成熟率(54.72%vs 32.08%)均高于对照组(均P<0.05)。结论：维持性血液透析患者自体动静脉内瘘术后采用束臂功能锻炼可有效促进动静脉内瘘成熟，缩短内瘘首次使用时间。更多还原

【Abstract】 Objective: To explore the clinical effect of bundle arm functional exercise in maintenance hemodialysis(MHD) patients after autogenous arteriovenous fistula operation. Methods: A total of 106 patients who were treated with MHD in Yichang Central People’s Hospital from January 2019 to December 2020 were selected as the subjects. According to the random digital table method, 106 patients were divided into the control group and the observation group, with 53 cases in each group. Patients in the control group were given routine fist holding exercise, and the observation group were treated with bundle arm function exercise. The maturity rate of internal fistula, the results of ultrasound examination of internal fistula, the use of internal fistula and the incidence of internal fistula related complications were compared between the two groups. Results: The internal fistula blood flow and vascular diameter of internal fistula in the observation group were higher than those in the control group at 2 months after operation(both P<0.05). The distance between internal fistula vessels and skin, the first use time of internal fistula and the hemostasis time after needle removal in the observation group were lower than those in the control group(all P<0.05). The clinical maturity rate(56.60% vs 35.85%)and ultrasonic maturity rate of fistula(54.72% vs 32.08%) in the observation group were higher than those in the control group(both P<0.05). Conclusion: After the operation of autogenous arteriovenous fistula in MHD, the implementation of bundle arm functional exercise can effectively promote the maturity of arteriovenous fistula, shorten the first use time of internal fistula.更多还原
