24例新生儿新型冠状病毒Omicron变异株感染的临床特征分析

【摘要】 目的 分析新生儿新型冠状病毒Omicron变异株感染病例的临床特点。方法 收集新型冠状病毒Omicron变异株感染新生儿24例为感染组,非感染组29例为对照组。分析感染患儿的临床特征及转归,比较两组患儿的炎症指标和其他实验室指标水平。结果 感染患儿在生后均有新型冠状病毒感染阳性家庭成员密切接触史。临床分型均为轻型,21例(87.50%)患儿出现发热,多伴有1种或几种其他症状。感染组结果显示4例(16.67%)白细胞计数降低,8例(33.33%)淋巴细胞计数降低;2例(9.52%)天冬氨酸氨基转移酶升高,21例(100%)前白蛋白均降低;5例(26.32%)胱抑素C升高;1例(4.76%)肌酸激酶升高,19例(90.48%)乳酸脱氢酶升高。与对照组比较,感染组血白细胞、淋巴细胞、淋巴细胞/单核细胞比值(LMR)、前白蛋白水平均显著降低(P<0.01﹚,血小板/淋巴细胞比值(PLR)、天冬氨酸氨基转移酶、胱抑素C、肌酸激酶、乳酸脱氢酶水平均显著升高(P<0.05﹚。所有患儿对症治疗后均痊愈出院,住院时间中位数为5d。结论Omicron变异株感染新生儿病例表现为轻型,临床症状主要为发热,病程呈自限性。感染患儿多伴有淋巴细胞计数降低,肌酸激酶、乳酸脱氢酶水平升高,LMR、PLR异常及低前白蛋白水平对于新型冠状病毒(COVID-19)感染的早期诊断具有重要的临床价值。

【Abstract】 Objective To analyze clinical characteristics of neonates infected with Omicron variant of severe acute respiratory syndrome coronavirus 2(SARS-CoV-2).Methods 24 neonates infected with Omicron variant of SARS-CoV-2 were collected as the infection group, while 29 cases of non-infected subjects were selected as the control group. We analyzed clinical features and outcomes of infected children, and compared levels of inflammatory and other laboratory indicators between the two groups of children.Results All 24 infants had a history of exposure to confirmed family members with SARS-CoV-2 infection after birth. The clinical classification was mild, the symptoms included fever in 21 cases(87.50%), often accompanied by one or several other symptoms. Blood white blood cell counts decreased in 4 cases(16.67%) and lymphocyte count decreased in 8 cases(33.33%); Aspartate transaminase increased in 2 cases(9.52%) and prealbumin decreased in 21 cases(100%); Cystatin C increased in 5 cases(26.32%); Creatine kinase increased in 1 case(4.76%), and lactate dehydrogenase increased in 19 cases(90.48%). Compared with the control group, white blood cells, lymphocytes, lymphocyte/Monocyte ratio(LMR), prealbumin levels in the infection group were significantly decreased(P<0.01), while platelet/lymphocyte ratio(PLR), aspartate transaminase, cystatin C, creatine kinase, lactate dehydrogenase levels increased significantly(P<0.05). All infants were cured after symptomatic treatment, and the median hospital stay was 5 days.Conclusion The clinical manifestation is mild in neonates infected with SARS-CoV-2 Omicron variant, in which the clinical symptoms is mainly fever, and the course of disease is self-limiting. Most infected infants are accompanied by decreased lymphocyte count, increased levels of creatine kinase and lactate dehydrogenase, abnormal LMR, PLR, and low prealbumin levels. These results have important clinical value for the early diagnosis of Coronavirus disease 2019(COVID-19) infection.
