【摘要】 目的：探讨中西医结合治疗首发精神分裂症的临床疗效及其影响因素。方法：回顾性选取2018年1月～2019年12月本院收治的88例首发精神分裂症患者，均给予利培酮片和九味肝泰胶囊联用治疗3个月，采用阳性和阴性症状（PANSS）评分评价其临床疗效，以重复性成套神经心理状态测验（RBANS）评分、个人和社会功能量表（PSP）评分和功能大体评定量表（GAF）评价患者恢复情况，并采用Logistic多因素分析其独立影响因素。结果：患者的治疗总有效率为82.95%(73/88)，且有效组患者（治疗总有效）治疗后的RBANS评分、GAF评分、PSP评分与治疗前比较分别上升、下降和上升，均具有统计学意义（P<0.05）；单因素分析结果显示，饮酒史、吸烟史、合并高血压、治疗后血清叶酸水平、治疗后血清同型半胱氨酸（Hcy）水平等是首发精神分裂症患者临床疗效的主要影响因素，多因素分析结果显示，饮酒史、吸烟史、合并高血压和治疗后血清Hcy水平是独立危险因素（P<0.05,OR>0.05）。结论：中西医结合治疗首发精神分裂症疗效良好，可有效提高患者机体认知功能，其疗效的影响因素包括患者饮酒史、吸烟史、合并高血压和治疗后血清Hcy水平。更多还原

【Abstract】 Objective: To explore the clinical efficacy and the influencing factors of integrated traditional Chinese and western medicine in the treatment of first-episode schizophrenia. Methods: A total of 88patients with first-episode schizophrenia admitted to our hospital from January 2018 to December 2019 were retrospectively selected. All patients were treated with risperidone tablets and Jiuwei Gantai capsules for 3 months. The positive and negative symptoms(PANSS) scores were used to evaluate the clinical efficacy.The repeatable battery for the assessment of neuropsychological status(RBANS) score, personal and social performance scale(PSP) score and global assessment of tunction(GAF) were used to evaluate the patient recovery, and Logistic multivariate analysis was used to analyze each influencing factors independently.Results: The responser ate was 82.95 %(73/88) in total, and the RBANS score, FCA score and PSP score of the response group after treatment were statistically significant compared with those before treatment(P<0.05).The results of multivariate analysis showed that drinking history, smoking history, hypertension, serum folic acid level and serum homolysteine(Hcy) level after treatment were the main factors affecting the clinical efficacy of the patients with first-episode schizophrenia. Among them, drinking history, smoking history,hypertension and serum Hcy level after treatment were independent risk factors(P<0.05). Conclusion: The efficacy of integrated traditional Chinese and Western medicine is significant in the treatment of first-episode schizophrenia, which can effectively improve the body function of patients. The efficacymay get affected by the history of drinking, smoking, hypertension and serum Hcy level after treatment.更多还原
