【摘要】 目的 探讨阿帕替尼联合放疗治疗局部晚期胃癌的临床效果。方法 选取2019年1—12月于赣州市人民医院及南昌大学二附院治疗的60例局部晚期胃癌患者作为研究对象，按随机数字表法分为观察组与对照组，各30例。对照组进行调强适形放疗（IMRT）治疗，观察组采用阿帕替尼与IMRT联合治疗。比较两组临床疗效、无进展生存期（PFS）、总生存时间（OS）及不良反应。结果 观察组的疾病控制率（DCR）(86.67%)高于对照组（60.00%），观察组PFS为（9.41±2.38）个月、OS为（18.94±6.71）个月，均长于对照组的（6.12±2.04）个月、（15.24±5.49）个月，差异有统计学意义（P<0.05）；两组出现的不良反应多为Ⅰ～Ⅱ度，经处理后可控，患者耐受性好；观察组血细胞减少发生率（33.33%）高于对照组（10.00%），差异有统计学意义（P<0.05）。结论 局部晚期胃癌患者采用阿帕替尼与IMRT联合治疗可提高DCR，延长患者的PFS及OS，会增加血细胞减少发生率，经处理后可控，患者耐受性好。更多还原

【Abstract】 Objective To explore the clinical effect of Apatinib combined with radiotherapy in the treatment of locally advanced gastric cancer. Methods Sixty patients with locally advanced gastric cancer who were treated in Ganzhou People’s Hospital and the Second Affiliated Hospital of Nanchang University from January to December 2019 were selected and divided into two groups according to the random number table method, with 30 cases in each group.The control group was treated with intensity-modulated radiotherapy(IMRT), and the observation group was treated with Apatinib combined with IMRT. The clinical efficacy,progression-free survival(PFS), overall survival(OS) and adverse reactions were compared between the two groups. Results The disease control rate(DCR)(86.67%) of the observation group was higher than that of the control group(60.00%); the PFS of the observation group was(9.41±2.38) months, and the OS was(18.94±6.71) months, which were all longer than those of the control group(6.12±2.04) months and(15.24±5.49) months, with statistical difference(P<0.05); the adverse reactions in the two groups were mostly grade Ⅰ～Ⅱ, which were controllable after treatment and well tolerated by the patients; the incidence of cytopenias in the observation group(33.33%) was higher than that of the control group(10.00%), and there was a statistical difference(P<0.05).Conclusion Combined treatment of Apatinib and IMRT in patients with locally advanced gastric cancer can improve DCR,prolong PFS and OS, and increase the incidence of cytopenias. It is controllable after post-treatment and well tolerated by patients.更多还原
