依洛尤单抗在急性冠状动脉综合征患者中的应用现状及疗效的回顾性分析

【摘要】 目的:回顾性分析急性冠脉综合征患者应用依洛尤单抗的降脂效果和安全性。方法:选取2021年3月~2022年3月入住武汉亚洲心脏病医院心内科的103例急性冠脉综合征患者,根据是否应用依洛尤单抗分为观察组和对照组。比较两组患者的人群特征,及使用依洛尤单抗1个月和3个月的降脂效果和安全性。结果:与治疗前相比,观察组治疗1个月后的总胆固醇、低密度脂蛋白、三酰甘油、脂蛋白a等指标水平均有明显降低(P<0.05),且总胆固醇、低密度脂蛋白、脂蛋白a水平均明显低于对照组(P<0.05)。治疗3个月后,观察组患者的总胆固醇、低密度脂蛋白和脂蛋白a较治疗1个月时进一步降低(P<0.05),总胆固醇、低密度脂蛋白、脂蛋白a水平均明显低于对照组(P<0.05),且观察组低密度脂蛋白<1.8 mmol·L-1的患者比例达到100%。观察组患者两种治疗方案治疗3个月后血脂下降情况差异无统计学意义(P>0.05)。两组治疗期间均未出现明显不良反应,两组患者肝肾功能指标比较,差异无统计学意义(P>0.05)。结论:依洛尤单抗能明显改善急性冠脉综合征患者的血脂情况,显著降低低密度脂蛋白水平,具有较好临床疗效且安全性良好。

【Abstract】 Objective: To retrospectively analyze the lipid-lowering effect and safety of evolocumab in patients with acute coronary syndrome(ACS).Methods: From March 2021 to March 2022, 103 patients with acute coronary syndrome who were admitted to the Department of Cardiology of Wuhan Asian Heart Hospital were selected. According to with or without the treatment of evolocumab, the patients were divided into treatment group and control group. The population characteristics of the two groups and the lipid-lowering effect and safety after 1-and 3-month treatment of evolocumab were analyzed. Results: Compared with those before treatment, the total cholesterol(TC), low-density lipoprotein cholesterol(LDL-C), triglycerides(TG), and lipoprotein a [LP(a)] levels of patients in the treatment group were significantly decreased after one-month treatment(P<0.05). Moreover, the levels of TC, LDL-C and LP(a) in the treatment group were significantly lower than those in the control group(P<0.05). After 3-month treatment, the levels of TC,LDL-C and LP(a) in the treatment group were further reduced when compared with those after one-month treatment(P<0.05), and the levels of TC, LDL-C and LP(a) in the treatment group were significantly lower than those in the control group(P<0.05). The proportion of patients in the treatment group with LDL-C <1.8 mmol·L-1 reached 100%. There was no statistically significant difference in the decrease of blood liqids between the two treatment regimens in the obeservation group after 3 months of treatment(P>0.05). There were no significant adverse reactions during the 3-month treatment period, and there were no statistically significant differences in liver and kidney function indicators between the two groups(P>0.05). Conclusion: Evolocumab can improve the blood lipid profile of patients with ACS and reduce the level of LDL-C significantly, which shows good clinical efficacy and security.
