腔镜模拟训练器模块化分层教学体系在胸外科临床教学中的实践探索

【摘要】 目的 探讨腔镜模拟训练器在胸外科临床教学中的应用,针对不同年级和类别的学员建立模块化分层教学体系。方法 2019年1月~6月,选择在浙江大学医学院附属第二医院胸外科进行临床培训的65名学员为研究对象,分别有浙江大学医学院5年制临床医学专业2015级24名见习生、2014级12名实习生,浙江大学医学院附属第二医院招录的26名外科学规范化培训住院医师、3名胸外科专科进修医师。全体学员均参与腔镜模拟训练器的上机实践,并选取不同难度分层的训练模块进行操作。根据操作后的问卷调查和个人访谈评价不同训练模块的使用满意度,以此为依据将不同难度的训练模块建设为模块化分层教程。结果 所有学员对腔镜模拟训练器的实践操作练习均给予好评,较高年资的学员对简单的训练模块需求性不高,其对腔镜下缝合打结等临床实际应用的操作训练更有需求性。结论 腔镜模拟训练器的应用,能够改变临床教学中偏重理论而操作相对缺失的现状,受到各层级学员的欢迎;同时,建立难度递进的模块化分层教程,有助于针对不同层次的学员进行分层培训,提高相关实践能力。

【Abstract】 Objective To explore the application of endoscopic simulator in clinical teaching of thoracic surgery, and try to establish a suitable modular layered course for students of different grades and categories. Methods 24 senior interns majoring in clinical medicine of College of Medicine, Zhejiang University, 12 senior interns majoring in clinical medicine of College of Medicine, Zhejiang University, 26 surgical standardized training residents and three thoracic surgery specialists enrolled in The Second Affiliated Hospital of College of Medicine, Zhejiang University from January 2019 to June 2019 were invited to participate in the computer operation of endoscopic simulation trainer, and selected training modules with different difficulty levels for operation. According to the questionnaire survey and personal interview after operation, they were used as effective practice modules for effect feedback, and based on this, the training modules with different difficulty levels were built into modular layered tutorials. Results All students praised the practical operation of endoscopic simulator, but the students with higher seniority did not have high demand for simple training modules. Conclusions The application of endoscopic simulator can make up for the current situation of emphasizing theory and relatively missing operation in clinical teaching, and is more popular with students at all levels. At the same time, the establishment of modular courses with progressive difficulty can carry out hierarchical training and improvement for students at different levels.
