【摘要】 目的：对比不同入路方式在腹腔镜辅助右半结肠癌D₃根治术中的应用效果。方法：选取2018年6月-2021年6月在佳木斯市中心医院行腹腔镜辅助右半结肠癌D₃根治术治疗的126例患者作为研究对象，依据随机数字表法分为对照组和试验组，每组63例。其中对照组通过中间入路方式进行手术，试验组通过头侧入路方式进行手术。比较两组手术指标、右半结肠癌活力指标、术后并发症发生率、复发率、转移率及肿瘤标志物水平。结果：相比对照组，试验组总手术时间、中央淋巴结清扫时间、术后排气时间均明显更短，术中失血量明显更少（P<0.05）。术后7 d，两组右半结肠癌活力指标、肿瘤标志物较术前均降低，且试验组均低于对照组，差异均有统计学意义（P<0.05）。试验组并发症发生率为7.94%，略低于对照组的15.87%，但差异无统计学意义（P>0.05）。两组复发率和转移率比较，差异均无统计学意义（P>0.05）。结论：在腹腔镜辅助右半结肠癌D₃根治术中应用头侧入路方式能够明显改善手术指标、右半结肠癌活力指标，降低术后肿瘤标志物水平。更多还原

【Abstract】 Objective:To compare the effects of different approaches in laparoscopic assisted D₃ radical resection of right colon cancer.Method:A total of 126 patients who underwent laparoscopic assisted D₃ radical resection of right colon cancer in Jiamusi Central Hospital from June 2018 to June 2021 were selected as the study objects,and they were randomly divided into the control group and the experimental group.The control group was operated through the middle approach;the experimental group was operated through cephalic approach.The operative indexes,right colon cancer viability index,postoperative complication rate,recurrence rate,metastasis rate,and the levels of tumor markers were compared between two groups.Result:Compared with the control group,the total operation time,central lymph node dissection time,postoperative exhaust time in the experimental group were shorter,and intraoperative blood loss was less （P<0.05）.At 7 d after operation,the right colon cancer viability indexes and tumor markers in two groups were lower than those before operation,and the experimental group were lower than those in the control group,the differences were statistically significant （P<0.05）.The incidence of complications in the experimental group was 7.94%,which was slightly lower than 15.87% in the control group,but the difference was not statistically significant （P>0.05）.There were no significant differences in recurrence rate and metastasis rate between two groups （P>0.05）.Conclusion:The application of cephalic approach in laparoscopic assisted D₃ radical resection of right colon cancer can significantly improve the operative indexes and right colon cancer viability indexes,reduce the levels of postoperative tumor markers.更多还原
