间歇性鼻饲配合吞咽训练在脑卒中吞咽障碍患者中的应用研究
Study on application of intermittent nasal feeding combined with swallowing training in patients with post-stroke dysphagia
【摘要】 目的 探讨间歇性鼻饲配合吞咽训练在脑卒中吞咽障碍患者治疗中的应用效果。方法 88例脑卒中吞咽障碍患者为研究对象,依据随机数字表法分为观察组与对照组,每组44例。对照组采取常规鼻饲配合吞咽训练,观察组采取间歇性鼻饲配合吞咽训练。比较两组的吞咽功能评分、营养状况指标、腹胀发生率、误吸发生率、吸入性肺炎发生率、满意度。结果 治疗后,两组吞咽功能评分均高于本组治疗前,且观察组吞咽功能评分(8.19±2.07)分高于对照组的(6.05±1.89)分,差异均有统计学意义(P<0.05)。治疗后,两组白蛋白、血红蛋白均高于本组治疗前,且观察组白蛋白、血红蛋白分别为(40.95±2.74)、(109.94±2.87)g/L,高于对照组的(38.09±2.31)、(106.52±2.39)g/L,差异均有统计学意义(P<0.05)。观察组腹胀发生率2.27%、误吸发生率2.27%、吸入性肺炎发生率0均低于对照组的15.91%、18.18%、11.36%,差异均有统计学意义(P<0.05)。观察组总满意率为97.73%,高于对照组的84.09%,差异有统计学意义(P<0.05)。结论 在脑卒中吞咽障碍患者吞咽训练中实施间歇性鼻饲,可改善吞咽功能和营养状况,减少腹胀、误吸、吸入性肺炎等并发症的发生,有利于提高其满意度。
【Abstract】 Objective To discuss the application effect of intermittent nasal feeding combined with swallowing training in patients with post-stroke dysphagia. Methods A total of 88 patients with post-stroke dysphagia were divided into observation group and control group according to random numerical table, with 44 cases in each group. The control group received conventional nasal feeding and swallowing training, while the observation group received intermittent nasal feeding and swallowing training. Both groups were compared in terms of swallowing function scores, nutritional status indicators, incidence of abdominal distension, incidence of misaspiration, incidence of aspiration pneumonia, and satisfaction rate. Results After treatment, the swallowing function scores in both groups were higher than those before treatment in this group, and the swallowing function score(8.19±2.07) points in the observation group was higher than(6.05±1.89) points in the control group, and the differences were all statistically significant(P<0.05). After treatment, the albumin and hemoglobin in both groups were higher than those before treatment in this group; the albumin and hemoglobin in the observation group were(40.95±2.74) and(109.94±2.87) g/L, which were higher than(38.09±2.31) and(106.52±2.39) g/L in the control group; the differences were all statistically significant(P<0.05). The incidence of abdominal distension,misaspiration and aspiration pneumonia in the observation group were 2.27%, 2.27% and 0, which were lower than 15.91%, 18.18% and 11.36% in the control group, and the differences were all statistically significant(P<0.05). The overall satisfaction rate of the observation group was 97.73%, which was higher than 84.09% of the control group, and the difference was statistically significant(P<0.05). Conclusion Intermittent nasal feeding in swallowing training of patients with post-stroke dysphagia can improve the swallowing function and nutritional status, reduce the occurrence of complications such as abdominal distension, misaspiration and aspiration pneumonia, and improve their satisfaction.
