新型冠状病毒肺炎患者恢复期PBMC中SARS-CoV-2中和抗体检测分析

【摘要】 目的 分析新型冠状病毒肺炎(COVID-19)患者恢复期外周血单个核细胞(PBMC)中急性呼吸综合征冠状病毒-2(SARS-CoV-2)中和抗体的检测作用。方法 回顾性分析40例COVID-19患者的临床资料,对其血浆进行假病毒中和实验,筛选具有中和效果患者的PBMC,再对其进行记忆B细胞筛选,并分析SARS-CoV-2中和抗体检测结果。结果 此次筛选的中和抗体中获得5个中和效果较好以及亲和力高的抗体,为进一步检测阳性样本的中和作用是否为SARS-CoV-2特异性中和,对水疱性口炎病毒(VSV)、急性呼吸综合征冠状病毒(SARS-CoV)、中东呼吸综合征冠状病毒(MERS-CoV)进行检测。检测结果显示:编号为415、726、1151的样本阳性,于1︰30稀释度下,对于3种病毒具有微弱的抑制性,表明此中和为非特异性中和作用;编号130、642、1143的样品,对于2种病毒具有较弱的中和作用;采用1︰90、1︰270的梯度进行稀释时,所有样本检测结果均为阴性。结论 COVID-19恢复期患者PBMC中SARS-CoV-2中和抗体检测结果显示, PBMC中具有特异的抗SARS-CoV-2作用,对于临床早期诊断及治疗此病可提供可靠依据。

【Abstract】 Objective To analyze the effect of severe acute respiratory syndrome coronavirus 2(SARSCoV-2) neutralizing antibody detection in peripheral blood mononuclear cells(PBMC) of patients recovering from corona virus disease 2019. Methods The clinical data of 40 COVID-19 patients were retrospectively analyzed, and their plasma was subjected to pseudovirus neutralization assay to screen PBMC from patients with neutralization effect, and then their memory B cells were screened, and the results of SARS-CoV-2 neutralizing antibody assay were analyzed. Results 5 antibodies with better neutralization effect and high affinity were obtained from neutralizing antibodies screened this time. Vesicular stomatitis virus(VSV), severe acute respiratory syndrome coronavirus(SARS-CoV), and Middle East respiratory syndrome coronavirus(MERS-CoV) were tested to further detect whether the neutralization of positive samples was SARS-CoV-2 specific neutralization. The test results showed that the samples numbered 415, 726 and 1151 were positive and had weak inhibition for 3viruses at 1︰30 dilution, indicating that this neutralization was non-specific; the samples numbered 130, 642 and 1143 had weak neutralization for 2 viruses; when dilutions of 1︰90 and 1︰270 gradients were used, all samples tested negative. Conclusion The results of SARS-CoV-2 neutralizing antibody detection in PBMC of patients recovering from COVID-19 showed specific anti-SARS-CoV-2 effect in PBMC, which can provide a reliable basis for early clinical diagnosis and treatment of this disease.
