二甲双胍联合德谷胰岛素治疗血糖控制不佳2型糖尿病的效果观察

【摘要】 目的 探讨二甲双胍联合德谷胰岛素治疗血糖控制不佳2型糖尿病患者的临床效果。方法 110例血糖控制不佳2型糖尿病患者,随机分为研究组与对照组,各55例。对照组患者采用二甲双胍联合门冬胰岛素30治疗,研究组患者采用二甲双胍联合德谷胰岛素治疗。比较两组治疗前后血糖水平及治疗效果、胰岛素应用剂量、血糖达标时间、不良反应发生情况。结果 治疗后,研究组空腹血糖、餐后2 h血糖、糖化血红蛋白均低于对照组,差异有统计学意义(P<0.05)。研究组治疗总有效率为94.55%,高于对照组的80.00%,差异有统计学意义(P<0.05)。研究组胰岛素应用剂量为(19.52±4.15)U/d、血糖达标时间为(6.75±1.13)d,对照组胰岛素应用剂量为(23.14±3.98)U/d、血糖达标时间为(9.63±1.08)d。研究组胰岛素应用剂量少于对照组,血糖达标时间短于对照组,差异有统计学意义(P<0.05)。两组不良反应发生率比较,差异无统计学意义(P>0.05)。结论 相比于二甲双胍联合门冬胰岛素30治疗方案,给予血糖控制不佳2型糖尿病患者二甲双胍联合德谷胰岛素治疗能够取得更加显著的降糖效果。

【Abstract】 Objective To discuss the clinical effect of metformin combined with insulin degludec in the treatment of type 2 diabetes mellitus with poor glycemic control. Methods A total of 110 cases of type 2 diabetes mellitus with poor glycemic control were randomly divided into the research group and the control group, with 55 cases in each group. Patients in the control group were treated with metformin combined with biphasic insulin aspart 30, and patients in the research group were treated with metformin combined with insulin degludec. The blood glucose level before and after treatment, therapeutic effect, insulin dosage, time of blood glucose reaching the standard, and adverse reactions were compared between the two groups. Results After treatment, the fasting blood glucose, 2 h postprandial blood glucose, and glycosylated hemoglobin in the research group were lower than those in the control group, and the difference was statistically significant(P<0.05). The total effective rate in the research group was 94.55%, which was higher than 80.00% in the control group, and the difference was statistically significant(P<0.05). The insulin dosage in the research group was(19.52±4.15) U/d, and the time of blood glucose reaching the standard was(6.75±1.13) d, while the insulin dosage in the control group was(23.14±3.98) U/d, and the time of blood glucose reaching the standard was(9.63±1.08) d. The insulin dosage in the research group was less than that in the control group, and the time of blood glucose reaching the standard was shorter than that in the control group. All the differences were statistically significant(P<0.05). There was no statistically significant difference in the incidence of adverse reactions between the two groups(P>0.05).Conclusion Compared with metformin combined with insulin aspart 30, metformin combined with insulin degludec can achieve more significant hypoglycemic effect in type 2 diabetes mellitus patients with poor glycemic control.
