116例血流动力学不稳定骨盆骨折急诊救治过程和预后分析

【摘要】 目的 分析血流动力学不稳定骨盆骨折患者院前急救和院内急诊救治过程中损伤控制策略(DC)的应用价值,指导临床及时高效救治。方法 回顾性分析116例血流动力学不稳定骨盆骨折患者的病历资料,根据治疗方法不同分为观察组(31例)和对照组(85例)。观察组患者采用损伤控制策略予以救治,对照组患者采用常规救治模式。比较两组患者的救治结果。结果 观察组患者的死亡率16.13%(5/31)和并发症发生率25.81%(8/31)均低于对照组的35.29%(30/85)、47.06%(40/85),差异均有统计学意义(P<0.05)。结论 血流动力学不稳定骨盆骨折患者应用院前急救和院内救治损伤控制策略可行且安全有效,能迅速、有效地抢救患者生命,提高其生存率,并减少并发症的发生。

【Abstract】 Objective To analyze the application value of damage control(DC) strategies in pre-hospital and in-hospital emergency treatment of patients with hemodynamically unstable pelvic fractures, so as to guide clinical timely and efficient treatment. Methods The medical records of 116 patients with hemodynamically unstable pelvic fractures were retrospectively analyzed, and they were divided into an observation group(31 cases)and a control group(85 cases) according to the treatment method. Patients in the observation group were treated with damage control strategy, and patients in the control group were treated with conventional treatment. The treatment outcomes of the two groups were compared. Results The mortality rate of patients in the observation group was 16.13%(5/31) and the incidence of complications was 25.81%(8/31), which were lower than 35.29%(30/85) and 47.06%(40/85) in the control group, and the differences were all statistically significant(P<0.05).Conclusion It is feasible, safe and effective to apply damage control strategies in pre-hospital and in-hospital emergency treatment to patients with hemodynamically unstable pelvic fractures, which can quickly and effectively save patients’ lives, improve their survival rate, and reduce the occurrence of complications.
