静息全周期比值与血流储备分数的一致性及影响因素分析

【摘要】 目的 明确静息全周期比值(RFR)与血流储备分数(FFR)的一致性及不一致时的影响因素。方法 连续入选2021年1月至2021年11月于北京大学人民医院心内科行RFR及FFR检查的住院患者,将其分为RFR和FFR一致组(高RFR/高FFR或低RFR/低FFR)及不一致组(高RFR/低FFR或低RFR/高FFR),探讨RFR和FFR的一致性以及不一致时的影响因素。结果 共纳入139例患者的163处病变,其中RFR和FFR一致组106例(128处病变),RFR和FFR不一致组33例(35处病变),RFR和FFR的一致性为78.5%(相关系数r=0.754,P<0.001)。多因素Logistic回归分析显示急性冠状动脉综合征(OR 2.36,95%CI 1.22~2.19,P=0.025)和左前降支病变(OR 4.64,95%CI1.06~3.44,P=0.031)均是导致RFR和FFR不一致的独立影响因素。结论 RFR和FFR具有较高的一致性,急性冠状动脉综合征患者和左前降支病变是两者不一致的独立影响因素。

【Abstract】 Objective To determine the coherence between the resting full-cycle ratio(RFR)and blood f ractional f low reserve(FFR)and the independent predictors of discordance between RFR and FFR. Methods Inpatients who underwent RFR and FFR examination in the Department of Cardiology of Peking University People’s Hospital from January 2021 to November 2021 were continuously included. They were divided into RFR and FFR coherence group and discordance group. The coherence of RFR and FFR and the independent predictors of discordance were discussed. Results A total of 163 lesions of 139patients were enrolled,including 128 lesions of 106 patients in the RFR and FFR coherence group and 35 lesions of 33 patients in the RFR and FFR discordance group. The coherence of R FR a n d FFR w a s 78.5%(r =0.754,P <0.001). According to multivariate Logistic analysis, acute coronary syndrome(OR 2.36,95%CI 1.22–2.19,P =0.025)and anterior descending artery lesions(OR 4.64,95%CI 1.06–3.44,P =0.031)were independent predictors of discordance between RFR and FFR. Conclusions RFR and FFR have high coherence.Patients with acute coronary syndrome and anterior descending artery lesions are independent predictors of discordance between RFR and FFR.
