【摘要】 目的 总结使用国产双盘无膜双腰封堵器介入治疗低体重患儿粗长管型动脉导管未闭（PDA）的经验。方法 对2018年1月至2021年1月湖南省儿童医院心内科使用国产双盘无膜双腰封堵器介入治疗的体质量≤6 kg且PDA为粗长管型的患儿43例进行总结。总结术后并发症发生情况，对比术后1年与术前心胸比、左心室射血分数、左心室内径、左心房内径的变化情况。结果 43例患儿中除1例患儿术后当天出现封堵器脱落经外科取出外，其余42例患儿均封堵成功，其封堵器位置形态良好，未见封堵器移位，术后半年随访均未见明显残余分流，术后1年与术前对比心胸比[（0.540±0.037）比（0.613±0.029）,t=10.06,P<0.001]、左心室射血分数[（71.88±6.01）%比（67.47±8.42）%,t=2.76,P<0.001]、左心室内径（长轴位）[（27.50±5.46）mm比（31.38±6.55）mm,t=2.94,P<0.001]、左心房内径（四腔位）[（15.31±2.64）mm比（17.41±3.78）mm,t=2.95,P<0.001]均有所改善，差异均有统计学意义。结论 在把握手术适应证的前提下，国产双盘无膜双腰封堵器对于低体重患儿粗长管型PDA介入治疗相对安全有效。更多还原

【Abstract】 Objective To summarize the experience of interventional treatment of patent ductus arteriosus(PDA)with long and thick tube in low weight children with domestic non-membrane double waist occluder.Methods From January 2018 to January 2021,43 children who were treated with domestic non-membrane double waist occluder in our department were summarized. Their weight was less than 6 kg and PDA was thick and long tube type. The postoperative complications were summarized, and the changes of cardiothoracic ratio,left ventricular ejection fraction,left ventricular inner diameter and left atrium inner diameter 1 year after surgery were compared with those before surgery. Results In all the 43 cases,except for 1 case whose occluder fell off on the day after surgery and was removed by surgery,the rest 42 cases were successfully occluded. The occluder was in good position and shape and no displacement of the occluder was observed. No obvious residual shunt was found in the follow-up of half a year after surgery. There were statistically significant differences in cardiothoracic ratio[(0.540±0.037)vs.(0.613±0.029),t=10.06,P<0.001],left ventricular ejection fraction[(71.88±6.01)% vs.(67.47±8.42)%,t=2.76,P<0.001],left ventricular inner diameter[(27.50±5.46)mm vs.(31.38±6.55)mm,t=2.94,P<0.001]and left atrial inner diameter[(15.31±2.64)mm vs.(17.41±3.78)mm,t=2.95,P<0.001]. Conclusions On the premise of grasping the surgical indications, domestic non-membrane double waist occluder is relatively safe and effective for the interventional therapy of thick and long tubular PDA in low weight infants.更多还原
