磁共振表观扩散系数在早期子宫内膜癌肌层浸润深度中的意义
Diagnostic significance of magnetic resonance apparent diffusion coefficient on the depth of myometrial invasion in early endometrial carcinoma
【摘要】 目的 探讨早期子宫内膜癌中心与边缘的ADC值的差异,以及它们与病理分期及肿瘤肌层浸润深度的关系。方法 回顾性分析浙江省丽水中心医院2017年1月-2019年11月采用3.0T常规MRI和扩散加权成像(DWI)(b=0,800s/mm~2)检查的子宫内膜癌患者66例。由两位放射科医师分别对子宫内膜肿瘤的边缘及中心部位进行ADC值测量。统计分析早期子宫内膜癌中心及边缘的ADC值差异,以及病理分期Ia和Ib期肿瘤边缘和肿瘤中心的ADC值差异。结果 早期子宫内膜癌肿瘤的中心与肿瘤边缘的ADC值差异有统计学意义(P<0.05);Ia和Ib期肿瘤边缘的ADC值差异有统计学意义(P<0.05或P<0.01)。结论 Ⅰ期子宫内膜癌肿瘤边缘的磁共振表观扩散系数值在不同浸润深度中有差异。
【Abstract】 Objective To investigate the differences in ADC values at the center and edge of early endometrial cancer, and their relationship with the pathological stage and the depth of tumor myometrial invasion. Methods 66 patients with endometrial cancer were examined with 3.0T conventional MRI and diffusion-weighted imaging(DWI)(b=0,800s/mm~2) from January 2017 to November 2019. ADC values were measured at the margins and central sites of a group of endometrial tu-mors by two radiologists. Statistical analysis of ADC values between centers and margins of early endometrial cancer, as well as ADC values between margins and tumor centers of pathological stages Ia and Ib. Results ADC values differ between the center and the tumor edge of the early endometrial cancer tumors(P<0.05), and ADC values differ in the stage Ia and Ib tumors(P<0.05 or P<0.01). Conclusion MR apparent diffusion coefficient values at the tumor margins of stage I endometrial cancer differ at different invasion depths.
