椎动脉沟环的颈动脉CTA表现

【摘要】 目的 观察颈动脉CT血管造影下寰椎椎动脉沟环(PP)的形态学特征。方法 回顾性分析了2020年7月-2021年6月浙江省嘉兴市第二医院行颈动脉CTA检查的1239例患者影像学资料,总结分析其中出现的PP变异的影像学特征。总结PP的在CTA矢状位重建图像上的椎动脉沟高度(VAGH)、寰椎后弓高度(PAH)、椎动脉沟环长短径和V3段椎动脉直径(VAD),并计算椎动脉沟环面积(PPA)。结果 1239例中出现94例(7.6%)PP,双侧PP 28例(2.3%),单侧66例(5.3%),总计PP数122侧。122侧PP的VAGH值2.4~7.8mm,平均(4.86±1.04)mm;PAH值4.2~11.6mm,平均(7.95±1.54)mm;PPA值15.30~48.58mm2,平均(29.83±7.27)mm2;VAD值1.6~5.4mm,平均(3.57±0.84)mm。结论 寰椎椎动脉沟环作为一种解剖变异,在人群中较为常见,建议行该区域手术者术前行CTA检查评估排除手术禁忌证。

【Abstract】 Objective To observe the morphological features of atlas vertebral ponticulus posticus(PP)under carotid CT angiography. Methods The imaging data of 1239 patients who underwent carotid artery CTA examination in the Second Hospital of Jiaxing City from July 2020 to June 2021 were retrospectively analyzed,and the imaging characteristics of PP variants in them were summarized and analyzed. The vertebral artery sulcus height(VAGH),the posterior atlas arch height(PAH),the long and short diameters of the vertebral artery sulcus and the V3 segment vertebral artery diameter(VAD)on the CTA sagittal reconstruction images of PP were summarized,and the vertebral artery sulcus ring area(PPA)was calculated. Results Among 1239 cases,94 cases(7.6%)of PP occurred,28 cases(2.3%)of bilateral PP,and 66 cases(5.3%)of unilateral PP,with a total of 122 cases of PP. The VAGH value of PP on the 122 sides was 2.4~7.8mm,(4.86±1.04)mm average; PAH value was 4.2~11.6mm,(7.95±1.54)mm average;PPA value was 15.30~48.58mm2,(29.83±7.27)mm2 average; VAD The value is1.6~5.4mm,(3.57±0.84)mm average. Conclusions The atlas vertebral artery groove ring,as an anatomical variation,is relatively common in the population. It is recommended that patients undergoing surgery in this region should undergo preopera-tive CTA examination to exclude contraindications to surgery.
