【摘要】 目的 探讨不同类型上斜肌麻痹手术方式及其术后效果,为上斜肌麻痹患者个体化术式选择提供参考。方法 回顾性分析手术治疗的上斜肌麻痹患者113例(164眼)临床资料,分析术后眼位、眼球运动、代偿头位、Bielschowsky征和双眼视功能恢复情况。结果 113例(164眼)患者,治愈97例(85.8%),好转10例(8,9%),无效6例(5.3%),总有效率94.7%。术前存在代偿头位79例,术后69例头位恢复正常,较术前改善(P<0.05)。术后存在双眼立体视功能者由术前49例(43.4%)增加至69例(61.1%),差异有统计学意义(P<0.05)。结论 上斜肌麻痹患者要进行详细检查并精确诊断,根据患者不同临床表现制定个体化手术方式,达到矫正斜视、改善头位、恢复双眼视功能的目的。更多还原

【Abstract】 Objective To investigate different surgical treatments and therapeutic effects of different types of superior oblique palsy,so as to provide reference for the individualized selection of surgical treatments for patients with superior oblique paralysis.Methods Retrospective cases series analysis.113 patients(164 eyes) with superior oblique palsy who underwent surgery were reviewed.The recovery of vertical strabismus,eye movement,compensatory head posture,Bielschowsky sign and binocular visual function were compared and analyzed.Results Up to the last follow-up for 6～24 months,there were 97 cases recoverd(85.8%),10 cases improved(8.9%),and 6 cases invalid(5.3%).The total effective rate was 94.7%.There were 79 patients with compensatory head posture before surgery and 69 patients returned to normal postoperative.The patients with binocular stereopsis increased from 49 cases(43.4%) to 69 cases(61.1%),and the difference was statistically significant(P<0.05).Conclusions Patients with superior oblique palsy should be carefully examined and accurately diagnosed,and individualized surgical treatments should be formulated according to the different clinical manifestations of patients,so as to correct the strabismus,eliminate the compensatory head posture,and restore the binocular visual function.更多还原
