【摘要】 目的 探讨复方桐叶烧伤油联合负压封闭引流治疗蛇伤性溃疡的临床效果。方法 选取2020年6月至2021年6月赣州市人民医院收治的78例蛇伤性溃疡患者作为研究对象，采用简单随机化法将其分为对照组（39例）和观察组（39例）。对照组患者采用负压封闭引流术治疗，观察组患者采用负压封闭引流术与复方桐叶烧伤油联合治疗。比较两组的二期植皮时间、表皮细胞生长因子、碱性成纤维细胞生长因子、创面面积、革兰氏阴性菌率、革兰氏阳性菌率及不良反应发生情况。结果 观察组的二期植皮时间短于对照组，差异有统计学意义（P<0.05）；观察组治疗后的表皮细胞生长因子、碱性成纤维细胞生长因子高于对照组，差异有统计学意义（P<0.05）；观察组治疗后的创面面积小于对照组，差异有统计学意义（P<0.05）；观察组治疗后的革兰氏阴性菌率、革兰氏阳性菌率分别为17.95%、28.21%，低于对照组的38.46%、51.28%，差异有统计学意义（P<0.05）。治疗期间，两组患者均未发生明显不良反应。结论 对蛇伤性溃疡患者应用负压封闭引流术与复方桐叶烧伤油联合治疗能够控制创面细菌感染，有助于相关生长因子表达，促进创面愈合，其安全性好。更多还原

【Abstract】 Objective To investigate the clinical effect of Compound Folium Paulownieae Burn Oil combined with vacuum sealing drainage in the treatment of snakebite ulcer. Methods A total of 78 patients with snakebite ulcer who were admitted to the Ganzhou People’s Hospital from June 2020 to June 2021 were selected as the research subjects, and they were divided into the control group(39 cases) and the observation group(39 cases) by simple randomization method. The patients in the control group were treated with vacuum sealing drainage, and the patients in the observation group were treated with the combination of vacuum sealing drainage and Compound Folium Paulownieae Burn Oil. The second stage skin grafting time,epidermal cell growth factor, basic fibroblast growth factor, wound area, Gram-negative bacteria rate, Gram-positive bacteria rate and adverse reactions were compared between the two groups. Results The second stage skin grafting time in the observation group was shorter than that in the control group, and the difference was statistically significant(P<0.05). After treatment, epidermal cell growth factor and basic fibroblast growth factor in the observation group were higher than those in the control group, and the differences were statistically significant(P<0.05). The wound area of the observation group after treatment was smaller than that of the control group, and the difference was statistically significant(P<0.05). The rates of Gram-negative bacteria and Gram-positive bacteria in the observation group after treatment were 17.95% and 28.21%,which were lower than 38.46% and 51.28% in the control group, and the differences were statistically significant(P<0.05).During the treatment, no obvious adverse reactions occurred in the two groups. Conclusion The combined treatment of vacuum sealing drainage and Compound Folium Paulownieae Burn Oil in patients with snakebite ulcer can control the bacterial infection of the wound, benefit for the expression of related growth factors, and promote wound healing in good safety.更多还原
