抵抗素、和肽素、氧化型低密度脂蛋白在急性缺血性脑卒中患者中的表达及临床意义

【摘要】 目的 探讨抵抗素、和肽素、氧化型低密度脂蛋白(ox-LDL)在急性缺血性脑卒中患者中的表达及临床意义。方法 选取2019年6月至12月扬州大学附属常熟市第二人民医院收治的85例急性缺血性脑卒中患者作为急性缺血性脑卒中组,选取同期在江苏省扬州大学附属常熟市第二人民医院进行健康体检的85名健康人作为健康对照组。急性缺血性脑卒中组在发病24 h内采集静脉血检测血清抵抗素、和肽素、ox-LDL水平,比较两组血清抵抗素、和肽素、ox-LDL水平,绘制ROC曲线分析抵抗素、和肽素、ox-LDL及三个指标联合检测对急性缺血性脑卒中的预测价值。结果 急性缺血性脑卒中组患者抵抗素、和肽素、ox-LDL水平均高于健康对照组,差异有统计学意义(P<0.05)。中等、大面积脑梗死患者抵抗素、和肽素、ox-LDL水平高于腔隙性脑梗死患者,差异有统计学意义(P<0.05)。大面积脑梗死患者抵抗素、和肽素、ox-LDL水平高于中等面积脑梗死患者,差异有统计学意义(P<0.05)。中度、重度神经功能损伤程度患者抵抗素、和肽素、ox-LDL水平高于轻度神经功能损伤程度患者,差异有统计学意义(P<0.05)。重度神经功能损伤程度患者抵抗素、和肽素、ox-LDL水平高于中度神经功能损伤程度患者,差异有统计学意义(P<0.05)。抵抗素、和肽素、ox-LDL及三指标联合预测急性缺血性脑卒中的曲线下面积分别为0.850、0.949、0.921、0.986。结论 抵抗素、和肽素、ox-LDL水平在急性缺血性脑卒中患者中高表达,且与患者脑梗死面积大小、神经功能损伤严重程度有关,高水平的抵抗素、和肽素、ox-LDL是急性缺血性脑卒中的风险预测因子,三指标联合预测急性缺血性脑卒中的价值高。

【Abstract】 Objective To explore the expression and clinical significance of resistin, copeptin and oxidized low-density lipoprotein in patients with acute ischemic stroke. Methods A total of 85 patients with acute ischemic stroke who were admitted to Changshu Second People′s Hospital affiliated to Yangzhou University from June to December 2019 were selected as the acute ischemic stroke group. Eighty-five healthy people who underwent physical examination in Changshu Second People′s Hospital at the same time were selected as healthy control group. The acute ischemic stroke group collected venous blood within 24 hours of onset to detect serum resistin, copeptin, ox-LDL levels, and compared the serum resistin, copeptin, ox-LDL level, draw ROC curve to analyze the predictive value of resistin, copeptin, ox-LDL and the combined detection of the three indicators in acute ischemic stroke. Results The levels of resistin, copeptin and ox-LDL in acute ischemic stroke group were higher than those in healthy control group, the differences were statistically significant(P<0.05). The levels of resistin, copeptin and ox-LDL in patients with moderate and large area cerebral infarction were higher than those in patients with lacunar cerebral infarction, and the differences were statistically significant(P<0.05). The levels of resistin, copeptin and ox-LDL in patients with large cerebral infarction were higher than those in patients with moderate cerebral infarction, and the differences were statistically significant( P < 0. 05).The levels of resistin, copeptin and ox-LDL in patients with moderate and severe neurological impairment were higher than those in patients with mild neurological impairment, and the differences were statistically significant(P<0.05). The levels of resistin, copeptin and ox-LDL in patients with severe neurological impairment were higher than those in patients with moderate neurological impairment,and the differences were statistically significant(P<0.05). The area under the curve of resistin, copeptin, ox-LDL and their combination in predicting acute ischemic stroke were 0.850, 0.949, 0.921 and 0.986, respectively. Conclusion The levels of resistin, copeptin and ox-LDL are highly expressed in patients with acute ischemic stroke and related to the size of cerebral infarction and the severity of neurofunctional injury. High levels of resistin, copeptin and ox-LDL are risk predictors of acute ischemic stroke. The combination of three indexes has high value in predicting acute ischemic stroke.
