【摘要】 目的 探讨胃镜活检对胃部高级别上皮内瘤变的诊断价值。方法 选取2018年8月至2021年8月菏泽医学专科学校附属医院收治的82例确诊为胃部高级别上皮内瘤变患者作为研究对象，以手术病理切片检查为金标准，分析胃镜活检检查早期胃部高级别上皮内瘤变阳性检出率、发展期胃部高级别上皮内瘤变阳性检出率、总胃部高级别上皮内瘤变阳性检出率，并计算胃镜与手术病理切片的一致性。结果 胃镜活检检查早期胃部高级别上皮内瘤变的阳性检出率为50.00%(41/82)、发展期胃部高级别上皮内瘤变阳性检出率为47.56%(39/82)、总胃部高级别上皮内瘤变阳性检出率为97.56%(80/82)，与手术病理切片检查[51.22%(42/82)、48.78%(40/82)、100.00%(82/82)]比较，差异无统计学意义（P>0.05）。接受胃镜活检诊断胃部高级别上皮内瘤变与手术病理切片的一致性经检验Kappa=0.833，一致性较好。结论 胃部高级别上皮内瘤变患者接受胃部活检检查后，结果与手术病理切片检查结果一致，临床诊断价值高，值得推荐。更多还原

【Abstract】 Objective To investigate the diagnostic value of gastroscopic biopsy in gastric high-grade intraepithelial neoplasia. Methods A total of 82 patients diagnosed with gastric high-grade intraepithelial neoplasia who were admitted to the Affiliated Hospital of Heze Medical College from August 2018 to August 2021 were selected as the research subjects. The positive detection rate of early gastric high-grade intraepithelial neoplasia, developmental gastric highgrade intraepithelial neoplasia and total gastric high-grade intraepithelial neoplasia were analyzed using surgical pathological biopsy as the gold standard, and the consistency between gastroscopy and surgical pathological biopsy was calculated. Results The positive rate of early gastric high-grade intraepithelial neoplasia was 50.00%(41/82), that of developing gastric high-grade intraepithelial neoplasia was 47.56%(39/82), and that of total gastric high-grade intraepithelial neoplasia was 97.56%(80/82). There were no significant differences compared with surgical pathology(51.22%[42/82], 48.78% [40/82], 100.00% [82/82]). The consistency between the diagnosis of high-grade intraepithelial neoplasia of the stomach by gastroscopy biopsy and the surgical pathological section was tested with Kappa=0.833, indicating a good consistency. Conclusion The results of gastric biopsy in patients with gastric high-grade intraepithelial neoplasia are consistent with the results of surgical pathological biopsy, and the clinical diagnostic value is high, which is worthy of recommendation.更多还原
