α-硫辛酸升高血清硫化氢水平改善糖尿病周围神经病变的研究

【摘要】 目的 探讨α-硫辛酸(AL)升高血清硫化氢(H2S)水平改善糖尿病周围神经病变(DPN的机制。方法 选取2019年1~12月于河北医科大学第三医院内分泌科住院的T2DM患者231例,分为单纯T2DM组(T2DM,n=109)和合并DPN组(DPN,n=122)。DPN组再分为常规治疗亚组(Con,n=64)和AL治疗亚组(AL,n=58)。检测各组血清H2S水平及相关生化指标,进行神经系统症状评分(NSS)和密歇根糖尿病神经病变评分(MDNS),测量感觉神经传导速度(SCV)、运动神经传导速度(MCV)。结果DPN组血清H2S水平低于T2DM组[3.67(3.01,4.57)vs 4.00(3.32,5.11)μmol/L,P=0.016]。Spearman相关分析显示,DPN患者血清H2S水平与SCV、MCV呈正相关(P<0.05),与NSS、MDNS呈负相关,(P<0.05)。AL治疗2周后,AL亚组血清H2S水平变化值高于Con亚组[0.96(0.24,2.13)vs 0.34(0.08,0.93)μmol/L,P=0.001]。结论 AL可通过升高DPN患者血清H2S水平改善临床症状。

【Abstract】 Objective To explore whether alpha-lipoic(AL)acid ameliorates diabetic peripheral neuropathy(DPN)by elevating serumhydrogen sulfide(H2S)level.Methods 231 in patients with type2 diabetes mellitus(T2DM)in Third Hospital of Hebei Medical University from January 2019 to December2019 were recruited in this study and divided into T2DM group(T2DM,n=109)and DPN group(n=122).The DPN group was split into conventional therapy subgroup(Con,n=64)and AL subgroup(AL,n=58).Serum H2S level and other correlated biochemical indexes were measured. Neuropathy symptom score(NSS)and Michigan diabetic neuropathy score(MDNS)were evaluated. Sensory/motor nerve conduction velocity(SCV/MCV)were detected.Results The serum H2S level of DPN group was lower than that of T2DM group[3. 67(3. 01,4. 57)vs 4. 00(3. 32,5. 11)μmol/L,P=0. 016]. Spearman correlation analysis showed that serum H2S in patients with DPN was positively correlated with SCV and MCV(P<0. 05),and negatively correlated with NSS and mDNS(P<0. 05). After two weeks treatment,the increment of serum H2S level in AL subgroup was higher than that in Con subgroup[0. 96(0. 24,2. 13)vs 0. 34(0. 08,0. 93)μmol/L,P=0. 001].Conclusion AL could alleviate DPN by elevating serum H2S level.
