犬前列腺射频消融后体积的动态演变及意义

【摘要】 目的 观察犬前列腺射频消融后体积的动态演变规律并探讨其意义。方法 经直肠超声(TRUS)引导下对5只比格犬的前列腺行射频消融(RFA),在左右两侧叶各消融一个灶区,共计10个消融灶。其中两只犬分别在RFA后1 d和1周处死,剩余3只在3个时间段(消融即刻、消融后1周和1个月)完成经直肠超声造影(TR-CEUS)和磁共振(MRI)检查后处死,动态观察前列腺和消融灶体积的变化,并与病理结果进行对照分析。结果 TR-CEUS和增强MRI测量消融灶体积无明显差异。前列腺平均体积在RFA后的即刻(19.8±5.85)mL和1周(21.2±5.59)mL较消融前(17.1±5.66)mL明显增大,但消融后1个月恢复至术前水平(17.6±5.90)mL。RFA灶平均体积在1个月后减少了32%±6%(30 W/60 s)和42%±13%(30 W/120 s)。病理表明,犬前列腺经过RFA后呈现凝固性坏死,其中部分消融灶在1个月后发展成充满液体的空腔。结论 犬前列腺经过RFA后体积一过性增大,消融灶体积随时间逐渐减小,临床上应注意消融病灶评估时间。

【Abstract】 Objective To analyze the volume changes in prostate and its ablated regions after radiofrequency ablation(RFA) in canine, and explore the changing rule and its values. Methods Transrectal ultrasound(TRUS) guided RFA was performed in five beagle prostate. RFA were operated in left and right lobes of prostate in five beagles respectively, and produced ten ablated regions. Two canines were sacrificed at one day and one week after RFA respectively. The remaining three canines were sacrificed after transrectal contrast enhanced ultrasound(TR-CEUS) and magnetic resonance image(MRI) at three moments(before RFA, one week and one month after RFA). The volume of prostates and its ablated regions were calculated by three diameters via the TRUS and MRI, and then compared with pathology. Results The volume of ablated regions showed no significant difference between TR-CEUS and enhanced MRI. The prostatic volumes at immediately(19.8±5.85) mL and one week later(21.2±5.59) after RFA were larger than pre-RFA(17.1±5.66) mL, but returned to the preoperative level(17.6±5.90) mL one month later. However, the volume of ablated regions reduced by 32%±6%(30 W/60 s) and 42%±13%(30 W/120 s) at one month later. Pathology demonstrated coagulative necrosis in ablated regions immediately after RFA. And these regions then developed into cavity filled with liquid one month later. Conclusions The prostatic volume temporary enlarged after RFA, and the ablated regions volume decreased gradually with time. It should pay attention to the opportunity in evaluating the ablated region in clinical practices.
