常规超声及造影征象与甲状腺乳头状癌BRAF V600E基因突变的相关性及其临床意义
Correlation of BRAF V600E Mutation and Conventional Ultrasound and Contrast-enhanced Ultrasound Features in Papillary Thyroid Carcinoma and Its Clinical Significance
【摘要】 目的 分析甲状腺乳头状癌(PTC)的超声征象与BRAF V600E基因突变的相关性。方法 回顾性分析236例PTC患者的病例资料。根据BRAF V600E基因突变与否分为阳性组及阴性组,对比两组一般资料、常规超声及超声造影(CEUS)征象,分析BRAF V600E基因突变的独立相关因素。结果 BRAF V600E基因突变阳性组在微钙化、纵横比、被膜侵犯、增强后大小、增强模式上与阴性组对比有显著差异(P<0.05)。微钙化、纵横比≥1及造影后病灶较前增大是BRAF V600E基因突变的独立相关因素。结论 微钙化、纵横比≥1及造影后病灶较前增大与BRAF V600E基因突变有相关性,对早期发现高危PTC具有一定指导意义。
【Abstract】 Objective To analyze the correlation of BRAF V600 E gene mutation and ultrasound features in papillary thyroid carcinoma(PTC). Methods Clinical data of 236 PTC patients were retrospectively analyzed. According to BRAF V600 E gene mutation, patients were divided into positive group and negative group, general data, conventional ultrasound and contrast-enhanced ultrasound(CEUS) images between the two groups was compared. The independent correlation factors of BRAF V600 E gene mutation were analyzed. Results There were significant differences between positive group and negative group in microcalcification, aspect ratio, membrane invasion, the size of nodules after CEUS and enhancement mode(P<0.05). Microcalcification, aspect ratio ≥1 and the size of nodules become bigger after CEUS were independent correlation factors for BRAF V600 E gene mutation. Conclusions Microcalcification, aspect ratio ≥1 and the size of nodules become bigger after CEUS were correlated with BRAF V600 E gene mutation. These features have certain guiding significance for early detection of high-risk PTC.
