13例胎儿脐-门-体静脉分流与结局之间的关系

【摘要】 目的 探讨胎儿脐-门-体静脉分流(umbilical-portal-systemic venous shunts, UPSVS)与预后之间的关系。方法 回顾性分析13例UPSVS的超声特点并追踪随访至出生后。结果 我们共发现4例静脉导管-体静脉分流(ductus venosus-systemic shunt, DVSS),1例肝外门静脉-体静脉分流(extrahepatic portal-systemic shunt,EHPSS),2例脐-体静脉分流(umbilical-systemic shunt, USS),其中1例同时合并肝内门静脉-体静脉分流(intrahepatic portal-systemic shunt, IHPSS),7例肝内门静脉-体静脉分流(intrahepatic portal-systemic shunt, IHPSS),发现合并染色体异常7.7%(1/13),其他超声异常46%(6/13),心胸比增大46%(6/13),胎儿生长受限38.4%(5/13),水肿7.7%(1/13),活产77%(10/13)。结论IHPSS的发生率最高,预后也相对最好,DVSS及IHPSS出生后分流可能自发性闭合。

【Abstract】 Objective To investigate the relationship between congenital umbilical-portal-somatic vein shunts(UPSVs) and fetal outcome. Methods The ultrasonographic characteristics of 13 cases of UPSVS were retrospectively analyzed and followed up to postnatal.Results 4 cases of ductus venosus--systemic shunt(DVSS), 1 case of extrahepatic portal--systemic shunt(EHPSS), 2 cases of umbilical systemic shunt(USS),1 case of USS combined with intrahepatic portal-systemic shunt(IHPSS), 6 cases of intrahepatic portal-systemic shunt(IHPSS) were found.chromosomal abnormalities were found in 7.7%(1/13), other ultrasonic abnormalities in 46%(6/13), cardiothoracic ratio increase in 46%(6/13), fetal growth restriction in 38.4%(5/13), edema was observed in 7.7%(1/13) and live birth was observed in 77%(10/13).Conclusion The incidence of IHPSS is the highest and the outcome is the best. Shunt of DVSS and IHPSS can be closed spontaneously after birth.
