杭州市中医院2019—2021年环境卫生学及消毒效果监测分析

【摘要】 目的 了解杭州市中医院环境卫生学及消毒质量,为针对性开展消毒工作提供依据。方法 2019—2021年采用现场采样和微生物检测方法,对杭州市中医院相关部门的空气、物体表面、医务人员手、使用中消毒剂、内镜、治疗用水进行采样监测,并对监测结果进行分析。结果 2019—2021年共监测各类样本7599份,其中合格7189份,总体合格率94.60%,各年合格率分别为93.91%、94.34%、95.19%。不同年份监测样本合格率比较,差异无统计学意义(P>0.05);Ⅰ类环境空气合格率高于Ⅱ类环境和Ⅲ/Ⅳ类环境,差异有统计学意义(P<0.05);Ⅰ/Ⅱ类与Ⅲ/Ⅳ类环境物体表面合格率比较,差异无统计学意义(P>0.05)。卫生手监测中,工勤人员合格率低于医生、护士合格率,差异有统计学意义(P<0.001);透析用水消毒合格率高于口腔治疗用水和内镜终末漂洗水消毒合格率,差异有统计学意义(P<0.001)。其中,2021年空气合格率高于2020年和2019年,差异有统计学意义(P<0.05)。2019年医务人员手合格率高于2020年和2021年,差异有统计学意义(P<0.05)。结论 该医院近年来医院消毒质量稳步提升,但仍存在不足之处,应针对医务人员手、治疗用水等薄弱环节进行重点干预,提高消毒合格率。

【Abstract】 Objective To understand the environmental hygiene and disinfection quality of Hangzhou Hospital of Traditional Chinese Medicine to provide a basis for targeted disinfection work. Methods Field sampling and microbiological detection methods were used to monitor the air, object surface, medical staff’s hands, disinfectant in use, endoscopes and therapeutic water of relevant departments of Hangzhou Hospital of Traditional Chinese Medicine from 2019 to 2021, and the monitoring results were analyzed. Results A total of 7599 samples were collected during the year 2019 to 2021, and 7189 were eligible with the total qualified rate of 94.60%, the qualification rates in the three years were 93.91%, 94.34% and 95.19% respectively. There is no statistically significant difference between the pass rates of monitoring samples in different years(P>0.05); the pass rates of Class Ⅰ ambient air are higher than those of Class Ⅱ environment and Class Ⅲ/Ⅳ environment, and the difference is statistically significant(P<0.05);there is no statistically significant difference between the pass rates of Class Ⅰ/Ⅱ and Class Ⅲ/Ⅳ environmental object surfaces(P>0.05). In sanitary hand monitoring, the pass rate of workforce was lower than that of doctors and nurses, and the difference was statistically significant(P<0.001); the pass rate of dialysis water disinfection was higher than that of oral treatment water and endoscopy final rinse water disinfection, and the difference was statistically significant(P<0.001). Among them, the air pass rate in 2021 was higher than that in 2020 and 2019, with a statistically significant difference(P<0.05). The medical staff hand pass rate was higher in 2019 than in 2020 and 2021, with a statistically significant difference(P<0.05). Conclusion The disinfection quality of the hospital has been steadily improved in recent years, but there are still deficiencies, weak projects such as medical staff’s hands and therapeutic water should be the focus of intervention to improve the qualified rate of disinfection.
