低频重复经颅磁刺激治疗癫痫的疗效和安全性
Efficacy and safety of low-frequency repetitive transcranial magnetic stimulation in the treatment of epilepsy
【摘要】 目的 探讨低频重复经颅磁刺激治疗癫痫的疗效和安全性。方法 选取2020年4—11月符合标准的武汉市第一医院神经内科86例癫痫患者,分为AEDs组(原有AEDs治疗)和低频rTMS联合AEDs组(低频rTMS联合原有AEDs治疗),持续治疗4周后,随访3个月,观察患者治疗前后发作次数、视频脑电图检查及不良反应。结果 低频rTMS联合AEDs组的总有效率高于AEDs组(P<0.05)。低频rTMS联合AEDs组癫痫发作次数减少到50%用时短于AEDs组(P<0.05)。癫痫持续状态与部分发作的显效率在低频rTMS联合AEDs组明显高于AEDs组(P<0.05);低频rTMS联合AEDs组治疗后EEG与AEDs组相比异常程度明显减轻(P<0.05)。两组不良反应发生率差异无统计学意义(P>0.05)。结论 低频重复经颅磁刺激联合抗癫痫药物治疗可提高治疗效果,减少发作次数,且安全性高。
【Abstract】 Objective To investigate the efficacy and safety of low-frequency repetitive transcranial magnetic stimula tion(r TMS) in the treatment of epilepsy. Methods A total of 86 patients with epilepsy in the Department of Neurology of Wuhan First Hospital from April to November, 2020, who meet the standards were selected and divided into the antiepileptic drugs(AEDs) group and the r TMS combined with AEDs group, respectively receiving original AEDs treatment as well as low-frequency r TMS combined with original AEDs treatment. After 4 weeks of continuous treatment,they were followed up for 3 months and observed before and after treatment for the number of seizures, video EEG examination results and adverse reactions. Results The total effective rate in the r TMS combined with AEDs group was higher than that in the AEDs group(P<0.05). The time for the number of seizures to reduce to 50% in the r TMS combined with AEDs group was shorter than that in the AEDs group(P<0.05). The treatment efficiency for status epilepticus and partial seizures in the r TMS combined with AEDs group was significantly higher than that in the AEDs group(P<0.05). The abnormal degree of EEG after treat ment in the r TMS combined with AEDs group was significantly reduced compared with that in the AEDs group(P<0.05). There was no statistically significant difference in terms of incidence of adverse reactions between the two groups(P>0.05). Conclusion Low-frequency repetitive transcranial magnetic stimulation combined with antiepileptic drug therapy can improve the treatment effect and reduce the number of seizures with high safety.
