PPARγ的Pro12Ala基因多态性与妊娠糖尿病易感性的Meta分析

【摘要】 目的:探索过氧化物酶体增殖物激活受体(peroxisome proliferator-activated receptor gamma, PPARγ)的Pro12Ala基因多态性与妊娠糖尿病(gestational diabetes mellitus, GDM)发病风险的相关性。方法:计算机检索Cochrane、Pubmed、Embase、中国知网和万方数据库中关于PPARγ的Pro12Ala基因多态性与GDM相关性的病例对照试验,检索时间为建库至2020年6月。对纳入文献的基因型进行H-W平衡检验,并应用NOS进行发表偏倚分析。Meta分析采用Revman 5.2和Stata 13.0软件进行。结果:共纳入20篇文献,3808名患者和7848名健康对照。Meta分析结果显示,在显性(OR=0.62,95%CI=0.51~0.76,P<0.00001)、隐性(OR=0.27,95%CI=0.10~0.74,P=0.01)、等位(OR=0.59,95%CI=0.48~0.71,P<0.00001)、杂合子(OR=0.65,95%CI=0.53~0.80,P<0.0001)和纯合子(OR=0.18,95%CI=0.06~0.58,P=0.004)基因模型下,PPARγ的Pro12Ala基因多态性与亚洲人的GDM发病风险显著相关,然而这种相关性并未在高加索和中东人种中出现。结论:PPARγ的Pro12Ala基因多态性与亚洲人的GDM发病风险显著相关。

【Abstract】 Objective To explore the relationship between PPARr Pro12 Ala gene polymorphism of peroxisome proliferator-activated receptor gamma(PPARγ) and the risk of gestational diabetes mellitus(GDM).s and gestational diabetes(GDM). Methods Cochrane, Pubmed, Embase, CNKI and Wanfang database were searched for case-control trials on the relationship between PPARr Pro12 Ala gene polymorphisms and GDM. The retrieval time was from the establishment of the database to June 2020. The genotypes included in the literature were tested for H-W balance, and NOS was used for publication bias analysis. Meta-analysis was performed using Revman 5.2 and Stata 13.0 software. Results A total of 20 literatures, 3808 patients and 7848 healthy controls were included. Our meta-analysis results revealed that PPARr Pro12 Ala gene polymorphisms was significantly related with the risk of GDM in Asians under the dominant(OR=0.62,95%CI=0.51~0.76,P<0.00001), recessive(OR=0.27,95%CI=0.10~0.74,P=0.01), allele contrast(OR=0.59,95%CI=0.48~0.71,P<0.00001), heterozygote contrast(OR=0.65,95%CI=0.53~0.80,P<0.0001) and homozygous contrast(OR=0.18,95%CI=0.06~0.58,P=0.004) models. However, the above relationship was not found in Caucasus and Middle East populations. Conclusion PPARr Pro12 Ala gene polymorphisms was significantly associated with the risk of GDM in Asians.
