【摘要】 目的 探讨IL-17、IL-6的表达水平与肛周脓肿术后病情及预后的关系。方法 选取2020年10月至2021年10月我院肛肠科行肛周脓肿切开引流术的80例低位肛周脓肿患者为研究对象，术后无形成肛瘘患者共25例设为A组，形成肛瘘患者共55例设为B组。采用ELISA检测IL-17、IL-6水平，比较术后形成肛瘘患者和未形成肛瘘患者间的差异。结果 两组术前IL-17、IL-6水平无明显差异（P> 0.05）。术后第1天及术后第3天两组IL-17水平、B组IL-6水平均较术前有显著差异（P <0.05）,A组术后第3天IL-6水平与术前有显著差异（P <0.05）。B组术后第3天IL-17水平[114.36(101.61,138.53)pg/m L]高于A组的[78.27(68.80,84.00)pg/m L],B组术后第3天IL-6水平[0.80(0.66,0.99)pg/m L]高于A组的[0.24(0.01,0.36)pg/m L]，均P <0.001。两组术后第1天及术后第3天VAS评分均与术前有显著差异（P <0.05）。术后第1天两组间VAS评分差异无统计学意义（P> 0.05），而术后第3天两组间VAS评分差异有统计学意义（P <0.05）。两组患者术后1天、术后3天的创面分泌物比较，差异均有统计学意义（P <0.05）。结论肛周脓肿术后形成肛瘘的患者术前IL-17、IL-6呈高表达，且术后第3天血清的IL-17、IL-6表达水平以及疼痛评分、创面分泌物水平均高于未形成肛瘘患者。考虑IL-17、IL-6表达水平可能参与了肛周脓肿术后形成肛瘘的发生、发展、预后全过程。更多还原

【Abstract】 Objective To explore the relationship between the expression levels of IL-17 and IL-6 on postoperative condition and prognosis of perianal abscess. Methods From October 2020 to October 2021, 80 patients with low perianal abscesses who underwent perianal abscess incision and drainage in our hospital’s anorectal department were selected as the research objects. A total of 25 patients(31.25%) with no anal fistula after operation were set as group A, and a total of 55 patients(68.75%) with anal fistula were set as group B.ELISA was used to detect the levels of IL-17 and IL-6, and the procedures were compared. The difference between patients with anal fistula and those without anal fistula. Results There was no significant difference in preoperative IL-17 and IL-6 levels between the two groups(P>0.05). The levels of IL-17 and IL-6 in group B on the 1st and 3rd day after operation were significantly different from those before operation(P<0.05). There was a significant difference before(P<0.05). The level of IL-17 in group B was [114.36(101.61, 138.53) pg/mL] on the third day after operation, which was higher than that in group A,which was [78.27(68.80, 84.00) pg/mL] and the level of IL-6 in group B on the third day after operation was [0.80(0.66, 0.99) pg/mL] was higher than group A by [0.24(0.01, 0.36) pg/mL], all P<0.001. The VAS scores on the 1st day and the 3rd day after the operation in the two groups were significantly different from those before the operation(P<0.05). There was no significant difference in the VAS score between the two groups on the first day after the operation(P>0.05), but there was a statistically significant difference in the VAS score between the two groups on the third day after the operation(P<0.05). There were statistically significant differences in wound secretions between the two groups at 1 day and 3 days after operation(P<0.05). Conclusion Patients with anal fistula after perianal abscess had high expression of IL-17 and IL-6 before operation, and the expression levels of IL-17 and IL-6 in serum, pain score and wound secretion level on the 3rd day after operation were higher than patients without anal fistula, considering that the expression levels of IL-17 and IL-6 are involved in the occurrence, development and prognosis of anal fistula after perianal abscess surgery.更多还原
