【摘要】 目的 分析氯吡格雷联合替罗非班治疗对进展性卒中美国国立卫生研究院卒中量表(NIHSS)评分、炎症指标及血小板功能的影响。方法 2019年5月至2020年8月我院收治的127例进展性卒中患者，根据治疗方法不同分为研究组(n=68,氯吡格雷联合替罗非班治疗)和对照组(n=59,氯吡格雷治疗)。比较两组NIHSS评分、炎症指标[肿瘤坏死因子-α(TNF-α)、白介素-6(IL-6)、超敏-C反应蛋白(hs-CRP)]、血小板功能[血小板黏附率、血小板聚集率、血小板P选择素、血小板CD62p]及不良反应。结果 治疗后两组NIHSS评分、TNF-α、IL-6、hs-CRP水平及血小板黏附率、血小板聚集率、P选择素、血小板CD62p水平均下降，且研究组低于对照组(P<0.05);两组不良反应总发生率比较，差异无统计学意义(P>0.05)。结论 使用替罗非班联合氯吡格雷治疗进展性脑卒中，可改善患者NIHSS评分、血小板功能以及炎症指标水平，安全性高。更多还原

【Abstract】 Objective To analyze the effects of clopidogrel combined with tirofibantherapy on the score of National Institutes of Health Stroke Scale(NIHSS), inflammatory indexes and platelet function in progressive stroke.Methods One hundred and twenty-seven patients with progressive stroke treated in our hospital from May 2019 to August 2020 were selected. According to different treatment methods, the patients were divided into a study group treated with clopidogrel combined with tirofiban(n=68) and a control group treated with clopidogrel(n=59). NIHSS score and inflammatory indexessuch as tumor necrosis factor-α(TNF-α), interleukin-6(IL-6), high sensitivity-Creactive protein(hs-CRP), platelet function indexes such as platelet adhesion rate, platelet aggregation rate, platelet P-selectin, and platelet CD62 P as well as adverse reactionswere compared between the two group.Results After treatment, NIHSS score, levels of TNF-α, IL-6 and hs-CRP, platelet adhesion rate, platelet aggregation rate, P-selectin and platelet CD62 P were decreased on average in the two groups, and the all indexes in the study group were lower than those in the control group(P<0.05). There was no significant difference in the total incidence of adverse reactions between the two groups(P>0.05).Conclusion Tirofiban combined with clopidogrel can improve the NIHSS score, platelet function, and inflammatory indexes in patients with progressive stroke.
