【摘要】 目的 探讨出院计划服务对单髁膝关节置换术后患者的出院指导质量和出院准备度的影响。方法 选取我院2020年1～5月住院的单髁膝关节置换术患者42例为对照组，2020年6～10月住院的单髁膝关节置换术患者39例为试验组。对照组给予骨外科常规护理和出院宣教，试验组开展多学科团队的出院计划服务，比较两组在出院指导质量、出院准备度、一般自我效能感、出院后1个月内非计划再入院率等方面的差异。结果 试验组出院指导的实际获得内容、指导技巧和效果2个维度评分均高于对照组(P<0.05);出院准备度的疾病知识、出院后应对及总得分均高于对照组(P<0.05),一般自我效能感得分显著高于对照组(P<0.05);出院后1月内非计划再入院率试验组低于对照组(P<0.05)。结论 对单髁膝关节置换术患者提供出院计划服务可促进患者在住院期间掌握康复训练技能，提高出院指导质量及出院准备度，以保障从医院到家庭的顺利过渡，降低出院后不良事件发生，减少再入院风险，提高患者满意度。更多还原

【Abstract】 Objective To investigate the effect of discharge planning service on the quality of discharge guidance and discharge readiness for patients after unicondylar knee arthroplasty.Methods Forty-two patients with unicondylar knee arthroplasty in our hospital from January 2020 to May 2020 were selected as a control group. Another 39 patients with unicondylar knee arthroplasty from June to October 2020 were selected as an experimental group. The control group was given orthopedic surgery routine nursing and discharge education, and the experimental group launched a multidisciplinary team discharge planning service. The discharge guidance quality, discharge readiness, general self-efficacy, and unplanned readmission rate within one month after discharge were compared between the two groups.Results The scores of actual acquired content, guidance skills and effect of discharge guidance in the experimental group were higher than those in the control group(P<0.05). The disease knowledge, post-discharge coping and total score of discharge readiness in the experimental group were higher than those in the control group(P<0.05). The general self-efficacy score in the experimental group was also significantly higher than that in the control group(P<0.05). The rate of unplanned readmission within one month after discharge in the experimental group was lower than that in the control group(P<0.05).Conclusion The discharge planning services for patients undergoing unicondylar knee arthroplasty can promote patients to master rehabilitation training skills during hospitalization, improve the quality of discharge guidance and discharge readiness. In order to ensure a smooth transition from hospital to home, the service could reduce the adverse events after discharge and the risk of readmission so as to improve the patients′ satisfaction.更多还原
