【摘要】 目的 探讨并存病院前预处置体系在腹股沟疝日间手术中的应用价值。方法 回顾性分析2016年3月—2020年2月在上海市第一人民医院疝中心行日间手术的1 454例腹股沟疝患者，以2018年3月应用并存病院前预处置体系的时间点为界，分成非预处置组与预处置组，采集患者临床资料进行对比分析。结果 预处置组的平均年龄、≥60岁占比、美国麻醉医师协会分级Ⅲ级占比大于非预处置组，预处置组的并存病比例高于非预处置组，差异有统计学意义（P<0.05）。其他一般临床资料、围手术期指标、术后并发症情况两组比较差异无统计学意义（P>0.05）。结论 院前预处置体系可以在保障医疗安全的前提下将更多有并存病的腹股沟疝患者特别是老年患者纳入到日间手术，提高日间手术的覆盖面，提升医疗服务质量和效率。更多还原

【Abstract】 Objective To explore the application value of outpatient pretreatment system for comorbidities in ambulatory inguinal hernia surgery. Methods A total of 1 454 inguinal hernia patients who underwent ambulatory surgery at the Hernia Center of Shanghai General Hospital from March 2016 to February 2020 were retrospectively analyzed, and the time point of applying the outpatient pretreatment system for comorbidities in March 2018 was used as the boundary to divide the group into non-pretreatment and pretreatment groups,and the patients’ clinical data were collected for comparative analysis. Results The average age,proportion of ≥60 years old and ASA Ⅲ in the pretreatment group were greater than those of the non-pretreatment group,and the proportion of comorbidities in the pretreatment group was higher than that of the non-pretreatment group,and the difference was statistically different(P<0.05). According to the general clinical data,perioperative indicators,postoperative complications,there was no statistical difference between these two groups(P>0.05). Conclusion On the premise of ensuring medical safety,the outpatient pretreatment system can include more patients with comorbidities,especially elderly patients,into ambulatory surgery and improve the coverage of ambulatory surgery,and enhance the quality and efficiency of medical care.更多还原
