膝关节半月板桶柄样裂的MRI表现
MRI appearances of bucket-handle meniscus tear
【摘要】 目的:总结半月板桶柄样裂的MRI特征。方法:选取2017年5月—2022年7月北京市顺义区医院收治的45例膝关节半月板桶柄样裂(bucket-handle tear of the meniscus,BHT)患者的影像和临床资料,所有患者均行MRI检查,并在2个月内有关节镜结果。记录6个MRI征象,包括髁间窝碎片征、前角翻转征、双后交叉韧带征、双前交叉韧带征、空领结征、外周残半月板征。结果:内侧BHT患者中,出现髁间窝碎片的有17例(85.0%),前角翻转征的有8例(40.0%),双后交叉韧带征的有16例(80.0%),双前交叉韧带征的有0例(0.0%),空领结征的有15例(75.0%),外周残半月板征的有9例(45.0%),外侧BHT患者中,出现髁间窝碎片的有24例(96.0%),前角翻转征的有9例(36.0%),双后交叉韧带征的有5例(20.0%),双前交叉韧带征的有11例(44.0%),空领结征的有18例(72.0%),外周残半月板征的有15例(60.0%)。结论:膝关节半月板桶柄样撕裂具有典型的MRI特征,MRI对其诊断具有重要价值。
【Abstract】 Objective To summarize the MRI features of Bucket-Handle meniscus tear.Methods The imaging and clinical data of 45 patients with bucket-handle tear of the meniscus(BHT) of the knee joint who were admitted to Beijing Shunyi Hospital from May 2017 to July 2022 were selected.All patients underwent MRI examination.and have arthroscopic results within two months.The six MRI signs of the two groups were recorded,including intercondylar fossa debris sign,anterior horn turnover sign,double posterior cruciate ligament sign,double anterior cruciate ligament sign,absent bow tie sign,and peripheral residual meniscus sign.Results Among the medial BHT patients,there were 17 cases(85.0%) with intercondylar fossa debris sign,8 cases(40.0%) with anterior horn turnover sign,16 cases(80.0%) with double posterior cruciate ligament sign,0 case(0.0%) with double anterior cruciate ligament sign,15 cases(75.0%) with absent bow tie sign,9 cases(45.0%) with peripheral residual meniscus sign.Among the lateral BHT patients,there were 24 cases(96.0%) with intercondylar fossa debris sign,9 cases(36.0%) with anterior horn turnover sign,and 5 cases(20.0%) with double posterior cruciate ligament sign,11 cases(44.0%) with double anterior cruciate ligament sign,18 cases(72.0%) with absent bow tie sign,and 15 cases(60.0%) with peripheral residual meniscus sign.Conclusion The bucket-handle meniscus tear has typical MRI feature and MRI is of great value in its diagnosis.
