【摘要】 目的：探讨艾司氯胺酮复合舒芬太尼应用于腹腔镜下腹股沟斜疝疝囊高位结扎术患儿的效果。方法：选取2020年11月-2021年10月福建省福州儿童医院收治的81例行腹腔镜下腹股沟斜疝疝囊高位结扎术患儿为研究对象，按照随机数字表法将其分为观察组(n=41)和对照组(n=40)，对照组予以艾司氯胺酮麻醉，观察组予以艾司氯胺酮复合舒芬太尼麻醉。观察两组麻醉效果、术后相关情况[苏醒时间、麻醉后复苏室(PACU)滞留时间、苏醒期躁动量表(PAED)、儿童疼痛行为量表(FLACC)评分]、血流动力学波动情况(HR、MAP、RR)及不良反应发生率。结果：观察组睁眼体动时间短于对照组，镇痛持续时间长于对照组(P<0.05)；观察组苏醒时间、PACU滞留时间均短于对照组，PAED、FLACC评分均低于对照组(P<0.05)。麻醉前，两组HR、MAP、RR水平对比，差异均无统计学意义(P>0.05)；拔喉罩时，两组HR、MAP、RR均高于麻醉前，但观察组均低于对照组(P<0.05)。观察组不良反应发生率低于对照组(4.88% vs 22.50%，P<0.05)。结论：艾司氯胺酮复合舒芬太尼应用于腹腔镜下腹股沟斜疝疝囊高位结扎术患儿均可有效维持血流动力学稳定，抑制苏醒期躁动，减轻术后疼痛程度，但艾司氯胺酮复合舒芬太尼治疗效果及降低不良反应发生率更加凸显，具有良好的麻醉效果优势。更多还原

【Abstract】 Objective:To explore the effect of Esketamine combined with Sufentanil on children undergoing laparoscopic high ligation of oblique inguinal hernia sac.Method:Eighty-one children who underwent laparoscopic high ligation of indirect inguinal hernia sac in Fuzhou Children’s Hospital of Fujian Province from November 2020 to October 2021 were selected as the study subjects,according to random number table method they were divided into the observation group (n=41) and the control group (n=40).The control group was anesthetized with Esketamine,the observation group was anesthetized with Esketamine combined with Sufentanil.Anesthesia effect,postoperative related conditions[time to wake up,post anesthesia care unit (PACU) retention time,pediatric anesthesia emergence delirium (PAED),pain behavior scale for children (FLACC) score],hemodynamic fluctuation (HR,MAP,RR) and incidence of adverse reactions of two groups were observed.Result:The time of eye opening and body movement in the observation group was shorter than that in the control group,and the duration of analgesia was longer than that in the control group (P<0.05);the recovery time and PACU retention time in the observation group were shorter than those in the control group,and the PAED and FLACC scores were lower than those in the control group (P<0.05).Before anesthesia,there were no significant differences in HR,MAP and RR levels between two groups (P>0.05);when laryngeal mask was removed,HR,MAP and RR of two groups were higher than those before anesthesia,but the observation group were lower than those of the control group (P<0.05).The incidence of adverse reactions in observation group was significantly lower than that in the control group (4.88% vs 22.50%,P<0.05).Conclusion:Esketamine combined with Sufentanil can effectively maintain hemodynamic stability in children undergoing laparoscopic high ligation of indirect inguinal hernia sac,restrain waking agitation,reduce the degree of postoperative pain.However,the therapeutic effect of Esketamine combined with Sufentanil and the reduction of the incidence of adverse reactions are more prominent,it has the advantage of good anesthesia effect.更多还原
