【摘要】 目的：探讨涡轮钻辅助拔除智齿中应用龈缘袋状切口及角型切口的手术效果。方法：选取2020年4月-2021年5月麻城市人民医院收治的60例智齿患者，按照随机数字表法将患者分为对照组和观察组，每组30例。两组均采用涡轮钻辅助拔除智齿治疗，观察组采用龈缘角型切口，对照组实施龈缘袋状切口。比较两组手术效果。结果：观察组主观感受Ⅰ级率为56.67%，高于对照组的26.67%，主观感受Ⅱ级率为23.33%，低于对照组50.00%，差异均有统计学意义(P<0.05)。两组主观感受Ⅲ级率比较，差异无统计学意义(P>0.05)。观察组疼痛、肿胀程度、术中出血量及手术时间均优于对照组，差异均有统计学意义(P<0.05)。观察组并发症发生率为16.67%，低于对照组的43.33%，牙窝完整率为90.00%，高于对照组60.00%，差异均有统计学意义(P<0.05)。术后，观察组TNF-α、IL-6均低于对照组，差异均有统计学意义(P<0.05)。结论：对涡轮钻辅助拔除智齿患者采用龈缘角型切口术可有效提高牙窝完整性，改善患者主观感受及降低并发症风险。更多还原

【Abstract】 Objective:To investigate the effect of gingival margin pocket incision and angle incision in turbine drill assisted extraction of wisdom tooth.Method:A total of 60 patients with wisdom tooth admitted to Macheng People’s Hospital from April 2020 to May 2021 were selected,they were divided into control group and observation group according to random number table method,30 cases in each group.Both groups were treated with turbine drill assisted wisdom tooth extraction,the observation group was treated with gingival margin angle incision,and the control group with gingival margin pocket incision.The results of operation were compared between the two groups.Result:The rate of subjective feeling grade Ⅰ in the observation group was 56.67%,which was higher than 26.67% in the control group,and the rate of subjective feeling grade Ⅱ was 23.33%,which was lower than 50.00% in the control group,the differences were statistically significant (P<0.05).There was no significant difference in the subjective grade Ⅲ rate between two groups (P>0.05).The degree of pain,swelling,intraoperative blood loss and operation time in the observation group were better than those in the control group,the differences were statistically significant (P<0.05).The incidence of complications in the observation group was 16.67%,which was lower than 43.33% in the control group,and the molar completeness rate in the observation group was 90.00%,which was higher than 60.00% in the control group,the differences were statistically significant (P<0.05).Postoperative TNF-α,IL-6 in the observation group were lower than those in the control group,the differences were statistically significant (P<0.05).Conclusion:Gingival margin angle incision can effectively improve the integrity of tooth socket,improve patients’ subjective feeling and reduce the risk of complications in patients with wisdom tooth extraction assisted by turbine drill.更多还原
