经颅定位磁刺激联合言语训练治疗运动性失语的疗效观察
Effect of Transcranial Magnetic Stimulation Combined with Speech Training on Motor Aphasia
【摘要】 目的:探讨经颅定位磁刺激联合言语训练治疗运动性失语的临床疗效。方法:纳入研究的样本来源于2021年1月~2021年10月我院康复科具有脑卒中后运动性失语的患者,病例数是60例。采用随机数字表法,分为两组,每组30例。对照组单纯接受言语训练治疗,观察组言语训练基础上接受经颅定位磁刺激治疗。比较两组患者的治疗效果以及治疗前后汉语标准失语症检查量表评分、美国国立卫生研究院卒中量表(NIHSS)评分、日常生活交流能力测验量表(CADL-T)评分变化情况。结果:观察组治疗总有效率比较高,为90.00%vs 66.67%,P<0.05;观察组治疗后自发谈话评分(39.26±3.08 vs 21.12±2.77)、理解评分(128.14±12.57 vs 107.21±11.26)、命名评分(44.25±4.18 vs 29.97±3.64)、复述评分(62.65±5.25 vs 40.02±4.34)均高于同组治疗前(P<0.05);观察组治疗后自发谈话评分(39.26±3.08 vs 31.34±3.22)、理解评分(128.14±12.57 vs 120.33±11.86)、命名评分(44.25±4.18 vs 35.83±4.66)、复述评分(62.65±5.25 vs 51.31±6.07)均高于对照组(P<0.05);观察组治疗后NIHSS评分(6.21±1.95 vs 15.01±3.11)低于同组治疗前,CADL-T评分(103.37±10.08 vs 63.95±6.77)高于同组治疗前,差异显著(P<0.05);观察组治疗后NIHSS评分(6.21±1.95 vs 10.16±2.89)低于对照组,CADL-T评分(103.37±10.08 vs 80.45±9.73)高于对照组(P<0.05)。结论:经颅定位磁刺激联合言语训练治疗运动性失语疗效显著,能够有效改善患者语言功能、神经功能和日常生活交流能力,值得临床推荐。
【Abstract】 Objective:To investigate the clinical effect of transcranial magnetic stimulation combined with speech training on motor aphasia.Methods:A total of 60 motor aphasia patients after stroke were selected into this study.The motor aphasia patients came from rehabilitation department of Rehabilitation of people’s hospital of Yichun City from January 2021 to October 2021.The research objects were randomly divided into two groups with 30 cases in each group.The control group received speech training alone, while the observation group received transcranial magnetic stimulation on the basis of speech training.The therapeutic effects of the two groups were evaluated through the scores of the Chinese Standard Aphasia Examination Scale, the NIHSS and the CADL-T.Results:The total efficiency in the observation group was 90.00% vs the control group 66.67%(P<0.05).The score of spontaneous conversation was 39.26 ± 3.08 after treatment vs 21.12 ± 2.77 before treatment, understanding was 128.14 ± 12.57 after treatment vs 107.21 ± 11.26 before treatment, naming was 44.25 ± 4.18 after treatment vs 29.97 ± 3.64 before treatment and repetition was 62.65 ± 5.25 after treatment vs 40.02 ± 4.34 before treatment.The above post-treatment scores in observation group were significantly higher than those before treatment in the same group(P<0.05).The score of spontaneous conversation in observation group was 39.26 ± 3.08 vs 31.34 ± 3.22 in control, understanding score in observation group was 128.14 ± 12.57 vs 120.33 ± 11.86 in control, naming score in observation group was 44.25 ± 4.18 vs 35.83 ± 4.66 in control and repetition score in observation group was 62.65 ± 5.25 vs 51.31 ± 6.07 in control.The above post-treatment scores in observation group were significantly higher than those in the control group(P<0.05).The post-treatment NIHSS score(6.21 ± 1.95 vs 15.01 ± 3.11)was lower than that before treatment and the post-treatment CADL-T score(103.37 ± 10.08 vs 63.95 ± 6.77)was higher than that before treatment in the observation group significantly(P<0.05).After treatment, NIHSS score(6.21 ± 1.95 vs 10.16 ± 2.89)in the observation group was lower than that in the control group, and CADL-T score(103.37 ± 10.08 vs 80.45 ± 9.73)was higher than that in the control group(P<0.05).Conclusion:The therapeutic effect of transcranial magnetic stimulation combined with speech training on motor aphasia can effectively improve the patient’s language function, neural function and daily life communication ability and is worthy of clinical recommendation.
