【摘要】 目的 回顾性分析自身免疫性疾病患者抗核抗体（ANA）和抗核抗体谱（ANAs）检测结果，探讨两者联合检测对自身免疫性疾病的诊断意义。方法 收集2018-2021年在该院就诊并进行自身抗体检测的患者共2 495例，ANA采用间接免疫荧光法（IIF）检测、ANAs采用免疫印记法（LIA）检测。结果 2 495例标本总阳性率为25.01%，男性阳性率为13.37%，女性为32.17%，且女性高于男性，差异有统计学意义（P<0.05）。ANA(+)519例（20.80%）,ANAs(+)404例（16.19%），其中ANA（+）/ANAs(+)323例（12.95%）,ANA（+）/ANAs(-)196例（7.86%）,ANA（-）/ANAs(+)81例（3.25%）,ANA（-）/ANAs(-)1 895例（75.95%），与IIF-ANA法比较，LIA-ANAs法检测总符合率为75.31%，灵敏度为62.24%，特异度为95.00%;ANA核型以颗粒型（79.96%）、着丝点型（6.74%）、均质型（5.59%）为主；ANAS中抗体阳性率抗Ro52抗体（25.21%）、抗干燥综合征A抗体（22.14%）、抗干燥综合征B抗体（8.72%）、抗核糖核蛋白（8.36%）、抗SM抗体（7.89%）、抗双链DNA抗体（5.77%）、抗线粒体M2抗体（5.65%）、抗着丝点B蛋白抗体（5.54%）。结论 IIF-ANA与LIA-ANAs这2种方法检测各有优点，但并不完全一致，联合检测可以有效降低漏检率，对于自身免疫性疾病的早期诊断、鉴别诊断、疗效观察及预后判断具有重要意义。更多还原

【Abstract】 Objective To retrospectively analyze the detection results of anti-nuclear antibody(ANA and ANA spectrum(ANAs)in patients with autoimmune diseases(AID),and explore the diagnostic significance of the combined detection of ANA and ANAs for AID.Methods A total of 2 495patients who were treated in this hospital from 2018to 2021and tested for autoantibodies were collected.ANA was detected by indirect immunofluorescence(IIF)and ANAs by line immunoassay(LIA).Results The total positive rate of2 495specimens was 25.01%.Among them,the positive rate of males was 13.37% and that of females was32.17%.Females were higher than males,and the difference was statistically significant(P<0.05).In the2 495specimens,519cases were ANA(+)(20.80%),404cases were ANAs(+)(16.19%),323cases were ANA（+）/ANA(+)(12.95%),196cases were ANA（+）/ANAs(-)(7.86%),81cases were ANA（-）/ANAs(+)(3.25%),1 895cases were ANA（-）/ANA(-)(75.95%).Compared with IIF-ANA method,the total coincidence rate of LIA-ANAs method detected was 75.31%,the sensitivity was 62.24%,and the specificity was 95.00%.ANA karyotypes were mainly granular(79.96%),centromere(6.74%)and homogeneous(5.59%).The positive rate of antibody in ANAS was anti-Ro52antibody(25.21%),anti-Sjogren′s syndrome A antibody(22.14%),anti-Sjogren′s syndrome B antibody(8.72%),anti-ribonucleoprotein(8.36%),anti-SM antibody(7.89%),anti-double stranded DNA antibody(5.77%),anti-mitochondrial M2antibody(5.65%)and anti-centromere B protein antibody(5.54%).Conclusion IIF-ANA and LIA-ANAs have their own advantages,but they are not completely consistent.The combined detection can effectively reduce the missed detection rate,which is of great significance for the early diagnosis,differential diagnosis,efficacy observation and prognosis judgment of AID.更多还原
