妊娠不同时期甲状腺功能参考范围的建立
Establishment of Reference Ranges of Thyroid Function in Different Periods of Pregnancy
【摘要】 目的 建立济南章丘地区妊娠不同时期正常甲状腺功能参考范围。方法 选取2020年7月—2021年6月于济南市章丘区人民医院就诊的正常妊娠妇女511名,分为3组,孕早期(≤13+6周)180名,孕中期(14~27+6周)192名,孕晚期(28~40+6周)139名;选取孕前健康女性89名作为对照,测定血甲状腺素(FT4)、促甲状腺素(TSH)含量。结果 与非妊娠女性比较,妊娠早期血FT4明显上升,差异有统计学意义(P<0.05);至妊娠中期略有下降,但差异无统计学意义(P>0.05);妊娠晚期已处于较低水平,差异有统计学意义(P<0.05)。血TSH水平在妊娠早期急剧下降,差异有统计学意义(P<0.05);而后逐渐升高,妊娠中期时已略高于非妊娠期,但差异无统计学意义(P>0.05);孕晚期时明显高于非妊娠期,差异有统计学意义(P<0.05)。结论 建立本地区特异的妊娠不同时期甲状腺功能参考范围,可有效提高甲状腺疾病诊治准确性,减少误诊误治。
【Abstract】 Objective To establish the reference range of normal thyroid function in different periods of pregnancy in Zhangqiu area of Jinan.Methods 511 normal pregnant women attending the People’s Hospital of Zhangqiu District,Jinan,from July 2020 to June 2021 were selected and divided into 3 groups:180 in early pregnancy (≤13+6weeks),192in mid-pregnancy (14-27+6weeks) and 139 in late pregnancy (28-40+6weeks).89 healthy women before pregnancy were selected as controls,and blood thyroxine (FT4),thyrotropin (TSH),levels were measured.Results.Compared with non-pregnant women,the median blood FT4 increased in early pregnancy,the difference was statistically significant(P<0.05).There was a slight decrease by mid-pregnancy,but the difference was not statistically significant (P>0.05).It was already at a lower level in late pregnancy,and the difference was statistically significant (P<0.05).Blood TSH levels decreased sharply in early pregnancy,and the difference was statistically significant (P<0.05).Then it gradually increased,and by mid-pregnancy it was already slightly higher than in non-pregnancy,but the difference was not statistically significant (P>0.05).In late pregnancy,it was significantly higher than in non-pregnancy,and the difference was statistically significant (P<0.05).Conclusion Establishing a region specific reference range of thyroid function in different periods of pregnancy can effectively improve the accuracy of thyroid disease diagnosis and treatment and reduce misdiagnosis and mistreatment.
