不同时机口服阿司匹林及钙剂对子痫前期患者肾功能、内皮损伤及胎盘缺氧的分子机制研究
Molecular Mechanism of Oral Administration of Aspirin and Calcium at Different Times on Renal Function, Endothelial Injury and Placental Hypoxia in Patients with Preeclampsia
【摘要】 目的 研究不同时机口服阿司匹林及钙剂对子痫前期患者肾功能、内皮损伤和胎盘缺氧的影响。方法选取2019年12月—2021年12月该院收治的60例子痫前期患者作为研究对象,按随机数表法将患者分为两组,各30例。研究组患者在孕周≤16周时口服阿司匹林和钙剂治疗;对照组患者在孕周>16周时口服阿司匹林和钙剂治疗。观察两组患者肾功能、内皮损伤情况以及胎盘组织中凋亡分子表达情况。结果 研究组Scr、BUN以及24 h尿蛋白水平低于对照组,差异有统计学意义(P<0.05)。研究组ET-1和PTX3水平低于对照组,ADM和APN水平高于对照组,差异有统计学意义(P<0.05)。研究组Bcl-2表达水平(122.85±15.37)高于对照组,Caspase-3(52.34±5.96)和Caspase-8(79.86±8.53)表达水平均低于对照组,差异有统计学意义(t=7.152、12.702、8.206,P<0.05)。结论 子痫前期患者孕16周及之前给予阿司匹林和钙剂治疗效果较好,能有效改善患者肾功能和血管内皮功能,抑制胎盘缺氧,减少胎盘滋养细胞的凋亡,值得临床应用及推广。
【Abstract】 Objective To study the effect of oral administration of aspirin and calcium at different times on renal function, endothelial injury and placental hypoxia in patients with preeclampsia.MethodsA total of 60 patients with preeclampsia admitted to the hospital from December 2019 to December 2021 were included as research objects.The patients were divided into two groups with 30 cases in each group according to the random number table method.The study group were treated with oral aspirin and calcium when gestational age ≤ 16 weeks, the control group were treated with oral aspirin and calcium when the gestational age > 16 weeks. The renal function, endothelial injury and the expression of apoptotic molecules in placental tissue were observed in the two groups of patients.ResultsThe levels of Scr, BUN and 24 h urinary protein in the study group were lower than those in the control group, and the difference was statistically significant(P<0.05). The levels of ET-1 and PTX3 in the study group were lower than those in the control group, and the levels of ADM and APN were higher than those in the control group, and the differences were statistically significant(P<0.05). The expression level of Bcl-2 in the study group(122.85±15.37) was higher than that in the control group, and the expression levels of Caspase-3(52.34±5.96) and Caspase-8(79.86±8.53) in the study group were lower than those in the control group, and the difference was statistically significant(t=7.152,12.702, 8.206, P<0.05).ConclusionPreeclampsia patients treated with aspirin and calcium before 16 weeks of gestation have better therapeutic effects, can effectively improve renal function and vascular endothelial function, inhibit placental hypoxia, and reduce the apoptosis of placental trophoblastic cells, which is worthy of clinical application and promotion.
