【摘要】 目的 探讨cN0单侧甲状腺乳头状癌根治术中止血纱或止血膜的应用效果。方法 选取安徽理工大学第一附属医院2020年1—9月行cN0单侧甲状腺乳头状癌根治术患者117例，随机分为止血纱组（n=38）、止血膜组（n=39）和对照组（n=40）。比较各组患者术后并发症、住院费用、住院天数、引流液中全段甲状旁腺激素（dPTH）水平、术前和术后血清C反应蛋白（CRP）水平、术后引流液量及引流管拔出时间（拔管时间）的差异。结果 各组患者手术时间、术中出血量、住院天数和术后并发症相比差异均无统计学意义（P均>0.05）。止血纱组和止血膜组住院费用均多于对照组（P均<0.05）。各组患者术后第1天和第2天dPTH水平及手术前后CRP水平差异均无统计学意义（P均>0.05）；各组患者术后第1天dPTH水平均高于同组术后第2天（P均<0.05）；术后第3天复查CRP较同组术前均升高（P均<0.05）。结论 在精细操作止血彻底情况下，cN0单侧甲状腺乳头状癌患者根治术中可不使用可吸收止血纱或止血膜。更多还原

【Abstract】 Objective To investigate the application effect of hemostatic gauze or hemostatic membrane in radical operation of c N0 unilateral papillary thyroid carcinoma. Methods A total of 117 patients with c N0unilateral papillary thyroid cancer undergoing radical operation in the First Affiliated Hospital of Anhui University of science and technology from January 2020 to September 2020 were randomly divided into three groups: hemostatic gauze group(n=38),hemostatic membrane group(n=39)and control group(n=40). The differences in postoperative complications,hospitalization costs,length of stay,total parathyroid hormone level in drainage fluid(dPTH),CRP,postoperative drainage fluid volume and drainage tube withdrawal time(extubation time)were compared among the three groups. Results There was no statistically significant difference between the three groups of patients in operation time,intraoperative blood loss,length of stay and postoperative complications(P all>0.05). The hospitalization expenses of the hemostatic gauze group and the hemostatic film group were higher than those of the control group(P all<0.05). Further comparison of dPTH on the first and second day after operation showed that dPTH on the second day after operation was significantly lower than that on the first day after operation(P all<0.05). CRP levels were higher on the third day after operation thanbeforeoperation(P all<0.05).Conclusion In thecaseofthorough hemostasisbyfinemanipulation,there is no need to apply hemostatic materials during radical resection of c N0 papillary thyroid carcinoma patients.更多还原
