【摘要】 目的 在桡骨远端骨折保守治疗中，可以采用肘上石膏固定或者肘下石膏固定，本研究的主要目的是探讨两种不同的固定方式对老年人桡骨远端骨折保守治疗效果的差异。方法 本研究的患者来源于本院急诊科，共有82名年龄在55岁以上（55-90岁）的桡骨远端骨折患者接受了治疗。这些患者根据随机数法分为两组，一组行骨折复位肘上石膏固定（A组），一组行骨折复位肘下石膏固定（B组），使用X线对比患者治疗前后的尺偏角、掌偏角和桡骨高度，以评估治疗情况。统计患者末次随访的改良Green-O’Brien腕关节评分，对患者预后情况进行评价。结果 结果表明，两组患者经不同的治疗，对其治疗前后的尺偏角、掌偏角和桡骨高度进行统计学分析，差异无统计学意义（P>0.05），但两组患者治疗后的腕关节评分存在差异，无统计学意义（P>0.05）。结论 对于桡骨远端骨折的保守治疗，肘下石膏和肘上石膏固定的治疗效果无较大的差异，但是肘下石膏不存在肘关节僵硬的风险，因此肘下石膏更有推广价值。更多还原

【Abstract】 Objective The main purpose of this study is to explore about the difference of two different fixation methods in the conservative treatment of distal radius fractures in the elderly patients. Methods The patients in this study were collected from the emergency department of our hospital. A total of 82 patients over 55 years old(from 55 to 90 years old)with distal radius fractures received orthopedic treatment. According to the random number method, these patients were divided into two groups, one group was treated with fracture reduction and plaster fixation above elbow(group A), and the other group was treated with fracture reduction and plaster fixation below elbow(group B). The ulnar deviation angle, palmar deviation angle and radius height before and after conservative treatment were compared by X-ray film to evaluate the effect of treatment. The modified Green-O’Brien wrist score at the last follow-up was calculated to evaluate the prognosis of the patients. Results The results showed that there were no significant differences in ulnar deviation angle, palmar deviation angle and radial height between two groups after different conservative treatment(P > 0.05), but there was no significant difference in wrist score between two groups(P > 0.05). Conclusions For the conservative treatment of distal radius fractures, there is no significant difference between the treatment effect of plaster fixation above elbow and plaster fixation below elbow, but there is no risk of elbow stiffness in infraelbow plaster fixation, so infraelbow plaster has better value for promotion.更多还原
